                                10                         name, address, or phone number.
                                11                                 ‘‘(B) Social security number.
                                12                                 ‘‘(C) Driver’s license number, passport
                                13                         number, military identification number, or other
                                14                         similar number issued on a government docu-
                                15                         ment used to verify identity.
                                16                                 ‘‘(D) Financial account number or credit
                                17                         or debit card number and any required security
