 Open a new Lake City Bank checking account at any one of our convenient
  office locations or online at www.lakecitybank.com.


 Stop using your old checking account. But don’t close your old checking
  account until all outstanding checks have cleared the account and your direct
  deposits/automatic payments are transferred to your new Lake City Bank checking
  account.


 Change any Automatic Deposits. Complete the attached Automatic Deposit
  Change Request Form to inform your employer or other organizations that you have a
  new Lake City Bank account. Fill out the form using your new Lake City Bank account
  number(s).


 Change any Automatic Payments. If you have any monthly payments
  automatically deducted from your account (e.g. car loan, insurance premium, utility
  bills, etc.), fill out the following Automatic Change Request Form to inform these
  companies of your new account information. Complete one form for each payment.


 Close your old account(s). Once your checks have cleared and your automatic
  payments have been changed to your new Lake City Bank account, submit the attached
  Request To Close Account Notice to your former bank to close your account(s).


 Take advantage of convenient Lake City Bank services like Internet
  Banking, Internet Bill Payment, and eStatements.
Use this form to notify your employer (or any other non-government organization that regularly sends a
payment to you) that you want the proceeds deposited directly into the Lake City Bank account specified
below. Some companies may require you to complete their own change form. Check with the company to

Enroll in Personal Internet Banking
      In your internet browser’s address bar, type: www.lakecitybank.com.
      Locate the Online Banking Secure Log-In box at the top right of home page. Click on the “Not
       registered for free Online Banking yet?” link.
      Follow the prompts to answer questions to complete the enrollment process. You may enroll
       with information from any of the following account types:
            o ATM or Debit Card (card number and PIN)
            o Checking, Savings or Money Market Account
                 (account number and last statement balance)
            o Loan (loan number, last payment amount and original principal amount)
            o Certificate of Deposit (account number)

Enroll in eStatements
      Log in to Personal Internet Banking.
      Click on the Statement link on the left sidebar menu of the Accounts Overview page
      Agree to the terms of the eStatement Service Agreement and Disclosure.
      For the first cycle following your eStatement enrollment, you will receive both a paper statement
       and an eStatement. Then the next statement will only be sent as an eStatement.
      Don’t forget, your eStatement delivery depends upon a valid e-mail address. If your e-mail
       address changes, please remember to update your information in the “Manage Contact
       Information” under the Customer Service tab.

Mutual Authentication Security Solution (MASS)
The next time you log in to Personal Internet Banking after your enrollment, you will be asked to select
Challenge Questions and a picture/passphrase for Mutual Authentication as an added layer of security
to protect your account information. Whenever you input your Username, we will display your selected
picture/passphrase so you know that you are accessing our internet banking site and not an impostor
site. If we do not recognize the computer being used when you are logging in, we will ask you to verify
your identity by answering two of your Challenge Questions. When you answer correctly, your selected
picture/passphrase will be displayed and you may continue logging in.
