First step in risk management is hazard identification. This
step include identify all hazards associated with the systems of
work.
You can ask these questions as your guideline;

Arethere a source of harm/potential loss ?
Who or what could be harmed ?
How could harm occur ?
Are there a source of harm at workplace ?

Potential hazard at workplace can occur from ;

       Substances
       Machines/tools
       Working methode
       Working environment
Who (or what) could be harmed ?

      Young/older workers
      Male/female
      Pregnant
      Third parties (contractor,guest,supplier,etc.)
      Etc.
How could harm occur ?

Could someone …………………?
1. Fall to (same or different level)
2. Struck by falling object
3. Struck by moving object
4. Caught in or between object
5. Exertion force
6. Contact with excessive temperature
7. Contact with electricity
8. Contact with hazardous substances
Hazard Identification Methode

Several methode can be applied to identify all hazards in
workplace such as;

                             inspection
                             survey
                             audit
                             questionare
                             data history
                             HAZOP, Fault Tree Analysis,etc.
Risk Analysis and Assessment

The next step in risk management is to analyse and
assess risk which occur from hazards in identification
process.
To do this task we can use parameters like; probability,
consequence and frequency.



     RISK = PROBABILITY x CONSEQUENCE or
 RISK = PROBABILITY x CONSEQUENCE x EXPOSURE

Probability
the chance of hazardous situations or event actually occuring or the
chance that a person will be harmed when they are exposed to
hazards.

-   The chance someone fall when walk across wet floor
-   The chance worker injured when contact with irritating substances
-   The chance worker get electrocuted when contact with electric wire
-   The chance worker struck by falling object when walking under
     scaffolding area
Risk Analysis and Assessment
Consequences
the outcome of an event or situation.

Could be rated as (to people);
- Fatality
- Major or serious injuries (dissability)
- Medium injuries (medical treatment)
- First aid
Exposure

The exposure is the frequency of occurrence of the hazardous
situation or frequency people expose to hazards (example;
frequency of driving a car over icy roads).
Ratings for exposure;
- Continuously (occur several times a day)
- Frequently (occur approximately one a day)
- Occasionally (occur once a week to once a month)
- Rarely (occur yearly to several years)
Risk Assessment


 Factors to be considered in risk assessment as follow;
  Details information about activities (duration, frequency,
   location dan person involved ?)
  Existing risk control in place
  Machines or tools
  Accident/incident statistics
  Literature or referrence
  Benchmark
  Expert judgement,etc

          ways to treat or control the risk. Risk treatment

 new work practice
 change in work environment
 new staff
