& If Applicable documentation at initial applicaiton:

                                                  Copy of all employment(s) income for most recent year:
                                                    ('10 W2's and/or 1099's and/or K1's)
                                                  Copy of most recent Tax Return - Federal Only:
                                                    ('10 Federal Tax Returns - All Schedules)
                                                  Copy of most recent pay stub(s) covering a 30 day consecutive period
                                                  Copy of additional verifiable source(s) of income:
                                                    (Part-time job, Social Security, Pension, Child Support, Alimony, etc…)
                                                  Copy of most recent Asset Account statement(s):
                                                    (Roth, 401k, Stocks, Bonds, Mutual Funds, Whole Life policy, etc…)
                                                  Copy of last months of consecutive Bank statement(s):
                                                    (Checking, Savings, Money Market, CDs, etc…)
                                                  Copy of Driver License or federally issued Identification Card:
                                                    (Sheriff ID Card, Passport, Visa, etc…)
                                                  Copy of most recent Mortgage Statement(s) for all properties owned
