Dear Parents,

Liberty Youth Baseball is doing a Yearbook Fundraiser, it will include team and individual player photos along with some
candid shots, advertisements from our sponsors and…...SHOUT OUTS! We are excited to offer to the families of our
players an opportunity to create a lasting memory for your baseball player. Placing a full color “Shout Out Ad” in the
book is an inexpensive way to make your player feel special and to support Liberty Youth Baseball. Yearbooks will be
sold for only $10 and must be pre-ordered. Thank You for your support of Liberty Youth Baseball!

Directions for photos:
Parents can submit any high resolution pictures on CD or by email (lybball@yahoo.com). If you are submitting your
photo on a CD please be sure to mark the CD with the child’s name and submit the CD with your order, please note that
the CD will not be returned. If submitting your photo by email please email your photo prior to submitting the form, you
will receive an email response verifying the receipt of the photo, please print that email response and include it with
your order form.

Payments:
We kindly ask for full payment at the time you submit your order. Payment is accepted by Cash, Check or Credit Card.

Yearbooks will be distributed near the end of May.
