Regional Faculty Applications
Every application, initial or renew, must be accompanied with the following:
    A personal letter explaining why you are applying
    Current CV/Resume
    Letter of recommendation from a current RF
    Copy of instructor card
    Teaching history
    Signed conflict of interest form

Incomplete applications will not be forward to the ECC Committee for review.

FYI
Please keep an eye out for mislabeled card packages. Some people have received mislabeled cards, i.e. label
says PALS Instructor Cards but actually PALS Provider cards. If this has happened to you please contact the
distributor you purchased it from.

AHA and OHSA Alliance
Under the Alliance, signed in November 2003, OSHA and the AHA will work together to raise awareness about
heart disease and stroke and develop programs to reduce death and disability from these conditions in the
workplace.

Goals of the Alliance include:

Developing training and education on AED programs

       Outreach and communication about prevention and training through the AHA training network and
        OSHA’s regional and area offices and state partners

       Training OSHA personnel in AHA’s Heartsaver AED and Heartsaver First Aid courses

       Nationally promoting workplace safety and health

about the “authorized provider logo” which was designed to promote the relationship between the American
Heart Association and the local trainer. It is available to all TCs and to any training site approved to offer
American Heart Association training. Please check out the logo information posted on the extranet site at
<http://exnet.heart.org> (this is a password protected site for TC Coordinators and Regional Faculty only; if you
have misplaced your username and/or password, please contact your Service Center).

What is the number of Sudden Cardiac Deaths each year?
From the 2005 Heart Disease and Stroke Statistics released in January 2005:
        The number American Heart Association uses is “about 335,000”, which refers to the annual
          number of people who die due to Coronary Heart Disease (CHD) in the emergency department or
          before reaching the hospital. Most of these deaths are caused by cardiac arrest resulting from
          ventricular fibrillation.
        Over 83% of those who die from CHD are age 65 years or older.
        The estimated average number of years of life lost due to heart attack = 11.5.
        50% of men and 64% of women who died from CHD had no previous symptoms of the disease.
        Sudden cardiac death accounts for 19% of sudden deaths in children between 1 and 13 years of
          age, and 30% between 13 and 21 years of age.
