                               Missing Information Checklist Sample


Dear Proprietary School Director:

Thank you for submitting your permit application for the term July 1 – June 30. Required information is
missing or incomplete from your application that must be completed before your permit is issued. Please
review the following checklist and remit all information that is incomplete or missing. Staff attests that all other
areas are complete.

    1. School Ownership-Articles of incorporation, including assumed name (this applies only to
    schools owned by Corporations). Call Bureau of Commercial Services at 517-241-6470. For
    schools not incorporated, include Doing Business As (DBA) certificate from county clerk.

    2. Specific curriculum for each program that includes performance objectives and program outlines
    showing units of instruction and clock hours of required instruction (specific tasks students will be
    required to perform). Identify what specific state or national standard curriculum is written. If not,
    state instructional purpose, occupational objective in catalog.

    3. A Personnel Qualification Form for each school administrator and instructor position.

    4. The School Catalog and School Catalog Information Form. See Sample Guideline Catalog 11.0

    5. The Business Plan for the First Three Years of Operation.

    6. Surety Bond payable to the State of Michigan in the amount of $5000.00 that matches legal
    name and address.

    7. The Fee Invoice and a certified check or money order payable to State of Michigan.

    8. A copy of the most recent fire inspection or certificate of occupancy is enclosed. (Note: Must be
    from a local government agency. Fire Extinguisher Reports, Sprinkler Reports, and Fire Inspection
    Reports with violations noted will not be accepted.)

    9. Copies of the School’s proposed advertisement including website materials.

    10. A copy (or sample) of the certificate or diploma to be issued to the student.

    11. A sample of the Student Transcript that includes the following information: a) Name of school,
    b) Address of school, c) Name of student, d) Address of student, e) Name of program, f) Grades of
    student, g) Date of each program enrollment, h) Date of completion and i) Number of credits or
    clock hours. See sample guideline 15.00 transcripts.

    12. A drawing or description of the instructional facility depicting the size, layout and equipment of
    programs specific classrooms and laboratories.

    13. A copy of the student contract and/or student enrollment application must include: “All tuition
    and fees paid by the applicant shall be refunded if the applicant is rejected by the school before
    enrollment. An applicant fee of not more than $25.00 may be retained by the school if the applicant
    is denied. All tuition and fees paid by the applicant shall be refunded if requested within three
    business days after signing a contract with the school. All refunds shall be returned within 30 days.


If the approval process is not complete due to the school failing to provide required application information
within 90 days from the date the initial application was received, the application will be closed and the applicant
will be required to submit both a new application and remit the required fee payments. In addition, the
application will be closed if no response is received within 30 days of the date any correspondence from this
office requesting additional information to complete the application process. Fees are due when the application
is delivered and are earned upon receipt. Fees submitted with applications that are closed cannot be applied to
subsequent applications. No refunds are granted under any circumstances. All new schools are subject to an
onsite inspection within one year of the date of issuance of the initial permit.
