					           CANCER PREVENTION, EARLY DETECTION, AND WELLNESS EDUCATION
                                  Ages 19 and older

Females:     Mammogram: Every other year from age 40 to 50,
                        every year after age 50.

              Pap Smear: Every year after age 19 or earlier if sexually active.

              Bone Density:Have this test starting at age 65 or earlier if your medical provider recommends it

Men:         PSA:           (Blood Test) Annually after age 50, after age 45 for African Americans

             Digital Rectal Exam: Annually after age 40

All:         Colorectal Cancer Screening: Fecal Occult Blood test annually after age 50 and three
                                          times per year if high risk.

             Vaccinations:
                   Tetanus-----------------Every 10 years
                   Hepatitis A & B-------If increased risk
                   Rubella:-----------------Nonimmune females of childbearing age and
                                             health care workers without evidence of immunity or prior
                                             immunization
                   Varicella--------------- If nonimmune adult
                   Influenza --------------Age 50 or over or high risk
                   Pneumococcal---------Age 65 or over or high risk

             Healthy Lifestyle:
                   Exercise 30 minutes per day three days per week
                   Eat a healthy diet including fruits, vegetables, and grains, decrease fat intake
                   Maintain a healthy weight
                   Have a physical exam by a medical provider at least annually
                   Limit alcohol consumption
                   Eliminate illegal drugs
                   Quit Smoking (Ask your provider for optional treatments)
                   Always wear sunblock when outdoors
                   Wear seatbelt/helmet
                   Put smoke detectors in your home
                   Keep medicines, firearms, chemicals and poisons out of reach of children


             Eyes/Teeth/Ears:
                   Have an annual eye exam, including glaucoma testing for high risk patients
                   Have a dental exam annually
                   Have a hearing test performed if hearing is problematic
