Sir,

         Sealed tenders are invited from the registered and experienced
companies/firms/socities for supply of manpower as well as Nurses/Paramedical Staff
turnkey services pertaining to different discipline and locations as per requirement of this
university. The tender forms can be obtained from this office on payment of Rs 5000/-
(non refundable) through Demand Draft payable to F.O. CSMMU Lucknow and can also
be downloaded from our website www.kgmcindia.edu. from 10-7-09 Detailed terms and
conditions can be seen at our website or can be obtained with tender forms from this office.
The tenders shall be accepted upto 31-7-09 on working days and working hours by
registered/speed posts only. The tenders should be addressed to the Registrar CSMMU
Lucknow.


       The Tenders will be opened on 10-8-09 at 3.00 p.m. by the Committee members
in presence of one authorised representative of the concerned firms, if present at that
time. Tender received after expiry of due date and time shall not be entertained.

                               Instruction to Tenderers


1-      Tenderer will have to submit tender in one separate sealed cover prescribed as
 below :-
2-      Earnest Money.
3-      Tender for providing Nurses & Paramdecal staff Technical Bid (one envelope)
 with this caption at the top of the envelope.
4-      Tender for providing Nurses & Paramedical staff Services (Custodial
 Cleaning)- Financial Bid (one envelope) with this caption at the top of the envelope.
                  i. The tenders downloaded from the internet must accompany a DD
                      of
5-      Rs 5000/- as tender form fee payable to Finance Officer, CSMMU Lucknow
 whereas the tender forms can also be purchases from the office of Finance Officer on
 payment of Rs 5000/- in the form of DD payable to Finance Officer, CSMMU.
                  i. Sealed Tenders will be opened on 10-8-09 at 03:00 p.m. in the
                      presence of Tenderer to examine the qualification criterion fixed
                      for approving the agencies.
                  ii. Earnest money amounting to Rs. 50,000/- (Rupees Fifty Thousand
                      only) in form of Bank Draft/Demand Draft will have to be
                      enclosed along with each tender in favour of Finance Officer,
                      Chhatrapati Shahuji Maharaj Medical University, Lucknow. In the
                      absence of EMD tender will not be considered and will be liable
                      to be rejected.
                 iii. All pages of the tender form and questionaire must be signed by
                      the Tenderers and submitted with Technical bid. Technical Bid
                      will be opened on 11-02-2009 at 02:00 pm in the presence of
                      Tenderer/Contractor. The Tender Open Committee will examine
                      all the documents related to this Tender. After scrutiny of
                      Technical Bid., the committee will decide whose firm/contractor
                      will qualify for opening these financial bid.
6-      Tenderer will have to submit along with documentary evidences for the
 registrations and licenses obtained as below:-
7-         Registration Under Shops and Commercial Establishment Act.
8-         Registration with Labour Deapartment of State Govt. for contract Labour.
9-         Registration with Provident Fund Community regarding EPF/CPF for the
 employee.
10-        Tenderers are advised to submit their bids strictly as per terms and conditions
 of the Bid Documents and not to stipulate any deviation.
11-        Separate certificate of working experience from Govt. Institutions shall be
 required to be attached with the technical bid of the tender form along with copies of
 latest orders and clientage list.
                    i. Late Tenders shall not be accepted/received.
                   ii. In case of any unscheduled Holiday falling on the prescribed
                       closing or
12-        opening day of the Tender, the next working day will be treated as
13-        scheduled opening or closing day of the Tenders as the case may be.
14-        Demonstration of equipments & instruments & list of consumables with make
 regarding cleaning & housekeeping is compulsory on the date of demonstration will be
 informed on the same day after opening of technical bid.
15-        If the Tenderers give deliberately wrong information to create take conditions
 for acceptance of the Tender, the University reserves the right to reject such tenders
 without assigning any reason. Not more than one tender will be submitted by one
 Tenderer for the same work.
16-        The successful Tenderers shall be required to execute the formal contract in
 accordance with the requirement of U.P. Chhatrapati Shahuji Maharaj Medical
 University, Lucknow within one week of the issue of offer for contract and shall start
 the contract on scheduled date as specified in the offer.
17-        More than one agency may be empanelled.
18-        The Bidders are advised to read carefully all the terms and conditions of the
 Tender documents which will form part of the contract.
19-        The Chhatrapati Shahuji Maharaj Medical University, Lucknow, reserves the
 right to accept or reject any or all tenders at their sole discretion without assigning any
 reason.
20-      Solvency certificate of 50 lakh (issued not earlier than 3 months )shall be
 necessary along with tender form
21-      Character Certificate from District Megistrate/Tehsildaar        (No blocklisted
 history) (issued not earlier than 3 months )shall be necessary along with tender form
22-     The tender form will be rejected in the absence of earnest money.
23-     Legal action may be initiated against such tenderer in case any of the
 information submitted by the tenderer is found to be false at any stage of the contract.
24-     For any loss, damage, Injury to property the patients, their attendants or its
 employee’s property due to non performance or negligent performance of the task
 enumerated in the agreement by the Contractors or his employees, the Contractors will
 be liable to make good, the loss or damage/ injury. For this purpose Joint Enquiry will
 be held to establish the cause and responsibility of the loss/damage/injury and assess
 the monetary value of the loss damage/ injury. The decision of Registrar, C.S.M.
 Medical University, Lucknow shall be final in case of disagreement between the
 contractor’s representative and the enquiry officer.
25-     The financial bid shall open only those firms who qualified in the technical bid.
26-     The financial bid shall be rejected if the rates quoted by the firms/firms below
 the rates approved by Labour Department.
27-     Every liability of employees of the empanelled agency/the tenderer shall be
 exclusively borne by the tender such as, Provident fund, Minimum wages,
 compensation to the dead or hurt employees or any damage/loss to the university or its
 employees.
