This Coming Orientation
To all RNHeals batch 3, please bring the following this coming
orientation:
1.   Philhealth number
2.   Pag-ibig number
3.   3 pcs 1x1 pic
4.   Birth certificate
5.   For married include marriage contract and birth certificate dependent
6.   Photocopy of 2 valid ID

Philhealth
1. You will go to philhealth office and register so you can have philhealth number. You
only have to pay one quarter and doh will pay the rest later. This is so because last
batch we had a problem when someone is hospitalized Philhelth will ask a receipt of
payment and we dont have individualized payment but a bulk payment for the whole
batch of rnheals. So we require it this way.

2. With regards to acquiring TIN- we need that if possible get it now so that when you
get to cdo on march 27 sabay sabay kayong mag open nang atm for your salary. Go to
BIR get the forms fill it up, if they need certificate of employment you may use the
endorsement letter of Director to your mayor where all your names are written there
and when they need sa signature of an employer you bring the forms to cdo. I will be in
cdo from 19-23. I do not have chance to sign it anymore.

TIN application
     1. Bring na lang sa march 27 anf filled up forms no. 1902 sa BIR kay didto na lng
        pirmahan sa accountant ang lower part sa forms. Didto na pud daw e file sa
        cagayan ang pag ask sa TIN, mao na ang instruction ni Ma'am Cynthia Descallar
        few minutes ago.
     2. kadto nag iwan ug forms sa ako office ako last friday ako e balik sa inyo, get it
        from my office where you left it.
     3. March 27-30 orientation in CDO free food and hotel accommodation. If you
        choose to go to CDO on the 26th you may stay in the DOH dorm at DOH CHD X
        office carmen cdo near city hospital. The dorm payment I am still negotiating it
      that instead of paying 150 per head per night mahimong 75 na lang same with
      doh regular employees
   4. pls inform your respective DOH reps kung go mo cdo 26 para maka pa reserve
      ug space sa dorm manager
   5. Dont forget to bring 3 pictures 1x1 that will be use for BIR and ATM
   6. Palihug kung panabi sa inyo kauban rn heals about ani nga update

For the March 27-30 orientation very sure accommodation on 27-30. For 26 I
will clarify it with Mam Cynthia but if there is none you can stay in the dorm of chd.
Aircon then doon only 150/night, 75/night if you are a member of the DOH union.

      SA march 26 wala pa free accommodation sa hotel but you can stay in DOH
       Dormitory at DOH CHD X office at serena st. carmen, near City Hospital.
      All you have to do is inform your doh rep that you’re going by March 26 so we
       can reserve a place for you.
      For the payment, I am negotiating with the management that you will only pay
       75 per head per night like the doh regular employees.
      You will know by monday if ok na that 75 lang
