4. Insurance: All campers must have their own medical coverage. Arkansas Girls Basketball Camp provides extra coverage only after your insurance policy has been
utilized. Campers will not be accepted unless a photocopy of the camper’s insurance card is submitted with this registration form or at registration if online registration
