  5.    Copy of Completed Needs Assessment Form (MHMC CF 014)

COMPLIANCE PACKAGE

   2.   Copy of Final Updated Loan Application – Fannie Mae 1003 (to be executed at closing)
   3.   Final Fannie Mae 1008 (Lender & ECD)
   4.   Title Commitment
   5.   Copy of Good Faith Estimate
   6.   Copy of Social Security card(s) or Copy of Alien Registration card(s)
   7.   Copy of Driver’s license(s), State I.D.(s), or Birth Certificate(s)
  8.    List of all household owned assets (i.e., bank accounts, trust accounts, safety deposit boxes, money markets, bonds, 401k’s, capital
        investments and property appraisals)
  9.    Most current month’s bank statement(s) on all bank account(s)
  10.   MyHome MyCoast Income Calculation Worksheet (Lender and ECD Final)
  11.   Verification of Employment (VOE), current pay stubs (within 30 days) covering at least 30 days, and W2 for all household members
        age 18 and older
  12.   Proof of full-time student status for household members age 18 and older, i.e. Fee Statements, Registrar or Admission’s office
        verification. If full-time student and unemployed, signed Certification of No Income Form
  13.   Documentation of child support and/or alimony payments (i.e., divorce decree, withholding order, TANF verification)
  14.   Documentation of income benefits from any agency (i.e., statement of benefits from Social Security Administration, Pension
        Statement or unemployment commission)
  15.   Form 1040 Tax Return for last year
  16.   Copy of Appraisal (URAR)
  17.   GCRC Approved Closing Attorney Information and Wire Funds Transfer Form (MHMC 006)
  18.   Copy of HUD-1
  19.   Notification of Change Form (MHMC 003, if applicable. Will require new Needs Assessment as well)
  20.   Miscellaneous Documents

POST CLOSING PACKAGE

  1.    MyHome MyCoast Checklist (MHMC 001)
  2.    Copy of Executed First Mortgage Note
  3.    Copy of Executed First Mortgage Deed of Trust
  4.    Copy of Final Executed HUD-1
  5.    Copy of Warranty Deed
  6.    Copy of Homeowners Insurance Declarations Page/Wind & Hail Policy/Flood Policy (if applicable)
  7.    *Executed GCRC Notes, Executed GCRC Deeds, Title Insurance Policy for Renaissance – these should be sent directly to GCRC by
        the closing attorney; if closing attorney forwards them to the lender, please include these original items with the Post Closing
        Package.

ALL CLOSING DOCUMENTS MUST BE RECEIVED WITHIN FIFTEEN (15) DAYS OF CLOSING OR THE LENDER WILL BE
CHARGED A PENALTY OF $50.00 EVERY FIFTEEN (15) DAYS.
