Initial Registration Appointment Schedule

We request that you be here at least 10 minutes prior to your scheduled appointment.
If you need to change or cancel your appointment, please call 568-4374. We need to have 24
hours notice if you are unable to keep your appointment.
Please provide all of the following information at your appointment:
1. Shot records. (for each child 12 years and under) Child must be 6 weeks to use the
child development centers. FCC homes will take child at 4 weeks of age, with initial
immunizations.
2. Health Assessment. (if you do not have a health assessment 6 months old or less at the
time of registration you will have 30 days to bring a health assessment in)
3. If your child/children has any special need, such as asthma or and allergy, there will be
additional forms that will need to be completed, signed and stamped by a doctor. Please,
let an administrative assistant know so the forms can be supplied.
4. Sponsor’s current end of the month LES for weekly or part time CDC/SAS
programs if total household income qualifies you for a Category lower than
Category 6. For hourly programs if total household income falls at or below
Category I.
5. Spouse’s or significant other’s pay statement (if applicable see above). We
need to establish total household income.
6. If spouse or significant other is attending school we will need a current school schedule if
using Full Time Care.
7. Please provide Sponsor’s or Spouse’s AKO e-mail address , duty number , and unit
name and deployment order if applicable.
8. Dual/ Single Military must provide the name and phone number of the Short Term
Provider listed on the Family Care Plan DA 5305-R within 30 days or at the time of
registration.
9. Local emergency contacts. (must provide 2 contacts, first and last name, phone number,
and must be provided before the first day of care)
10. USDA forms must be completed for each child 12 years and under using CYS programs.
11. If you are registering your child for full-time care, the full payment will be due at the
time of the registration.
12. In order to enroll for any sport your child must have a sports physical on file
at the beginning of the sport’s season.
Central Registration Office for Child and Youth Services is located in Bldg. 1743 (between
the Fort Bliss Inn and the Hertz Car Rental Office, across the street from the Fort Bliss
Federal Credit Union on Victory Road)
Hours of Operation: 07:00-17:30 (7:00-5:30pm) M-F
