New Child U.S. Passport
If you purchased a New Child Passport from PassportMax.com, we'd like to help you get started in the right direction with the following requirements. Don't be overwhelmed. We are here to assist you each step of the way. Even though you made your purchase online for a New Child Passport, your work is not done. We still need your help in order to efficiently expedite your order. First, you will need to gather all of the required information (listed here) and then take them to your nearest acceptance agent to seal your documents and then send your package to us via Federal Express. There is one government form (called the DS-11) you will need to complete, as well as what we call an LOA (stands for Letter Of Authorization... it authorizes us to process your passport paperwork). When you send in your package, go ahead and use this document as your cover page. Let’s get started now by going ahead and filling out the following questions. You’ve already ordered Online, but completing these questions will help us match your order in our database.

Your Package Checklist:
First thing, complete “Your Information” (above). Simple enough! Second, complete, download & print 2 copies of the DS-11 Form from the government website. Here is the link: https://pptform.state.gov/FraudAbuseNotice.aspx?form After you submit that you have read the disclaimers on the first page, the second page will have 3 sections.* Click “Continue” beneath the option that reads “Apply for a Passport Book”. NOTE: Do NOT sign the
application form until a Passport Acceptance agent instructs you to do so. One copy of this form will go inside the envelope from the Acceptance Agent and the second copy will go outside of the sealed envelope.
LOA (Letter Of Authorization). You will find the LOA attached to this document (page 4). NOTE: Leave the second blank empty -it’s where you are authorizing us to handle the submission and pick-up of your passport documents (after they’ve been issued by the passport agency). NOTE: BOTH Parents must sign this for the minor applicant. We need a proof of your U.S. citizenship. This would be one of the following: ORIGINAL certified copy of U.S. Birth Certificate; or a previous valid U.S. passport; or Report of Birth Abroad (Form FS-240); or Certification of Birth Abroad (Form DS-1350); or Certificate of Citizenship or Naturalization from USCIS. NOTE: A certified birth certificate has a registrar’s raised, embossed,
impressed or multicolored seal, registrar’s signature and the date the certificate was filed with the registrar’s office; which must be within 1 year of your birth. Please note that some short (abstract) versions of birth certificates may not be acceptable for passport purposes.
We need ONE form of primary identification. This would be either: previously issued & undamaged U.S. passport; Naturalization Certificate; Valid Driver’s License; Current Government ID (city, state or federal) or Current Military ID (for military applicants and dependents). We need 2 passport photographs. Must be less than 6 months old, identical and 2x2 inches. We need a copy of your travel itinerary. A copy of either your airline ticket(s) or flight itinerary will serve as the required proof regarding your departure date. We need your $120 check made payable to the U.S. Dept. of State, as well as a $25 execution fee to the Acceptance Agent. These are government fees. Your payment to PassportMax was for the expediting of your passport processing, but the Federal Government still requires their standard fees for a New Child Passport. NOTE: Please indicate your Date of Birth in the memo field of
your check.
Take all of these documents to your nearest Acceptance Agent. While at the Agent, inform them that you will be using a courier service (that’s us!). The Acceptance Agent will stamp the envelope before you send it to us. Do not open the envelope after the Agent seals your document. Remember, a second copy of the Form DS-11 will go on the outside of the sealed envelope. BOTH parents must appear in-person with the minor applicant. If one parent cannot be present, s/he must fill out a Statement of Consent: Issuance of a Passport to a Minor (Form DS-3053).
