      HEALTH AND DISABILITY STATE PROGRAMS IN
                     NEBRASKA

Agency              Nebraska Department of Education’s Vocational
                    Rehabilitation (Voc Rehab)
Description         Voc Rehab serves people with disabilities except consumers
                    who are blind or visually impaired; these individuals are
                    served by the Commission for the Blind and Visually
                    Impaired. Voc Rehab staff provides services locally in
                    multiple offices throughout the state, as well as in schools,
                    one-stop centers, mental health centers, and other
                    community programs to provide services locally to eligible
                    Nebraskans. The Nebraska Assistive Technology Program
                    and the Disability Determinations Section are located within
                    Vocational Rehabilitation.

                    Community-Based Services
Description         Community-Based Services provide assistance to adults
                    with disabilities (ages 18 to 64), helping them meet their
                    daily needs for living in their own homes, including help with:
                    housekeeping, laundry, meals, paying bills, essential errands
                    and shopping, personal care, minor home repairs, personal
                    assistance services, respite for primary caregivers,
                    transportation to medical appointments or to community
                    services or grocery shopping, home delivered meals,
                    assistive technology/home modifications, independence
                    skills building, nutrition services, and personal emergency
                    response system; services for adults with cystic fibrosis,
                    hemophilia or sickle cell disease; assisted living service;
                    adult day services; adult family home services; hospice; and
                        home health. Services are also provided to children with
                        disabilities and their families such as child care, respite
                        services, assistive technology/home modification, nutrition
                        services, transportation services, funds for children’s
                        specialized health care, and independent skills building.
                        Mental health services to children and their families include
                        outpatient treatment, day treatment, respite care, and
                        school-based wraparound.

                        Division of Developmental Disabilities
Description             This division provides certification, technical assistance,
                        regulations and payment for providers of community-based
                        developmental disabilities services; operates a 24-hour
                        intermediate care facility for persons with developmental
                        disabilities; administers home and community-based
                        Medicaid Waivers; acts as a single point of entry for
                        specialized developmental disabilities services; operates a
                        statewide registry of people eligible for specialized
                        developmental disabilities services; provides training and
                        technical assistance statewide to staff of community-based
                        developmental disabilities service providers; and provides
                        short-term community-based consultation and residential
                        behavioral intervention and training.

Description             The Health Promotion Unit is responsible for the
                        Cardiovascular Health Program, Comprehensive Cancer

                        Prevention and Control Program, Nutrition and Activity for
                        Health, The Preventive Health/Health Services (PHHS)

Description             DPFS provides funding for services to individuals with
                        disabilities to help them continue to live independently or
                        help families stay together; services may include

                        transportation, special equipment, and home modifications.
