Seattle Cancer Treatment and Wellness Center is a patient-focused integrative care
facility dedicated to healing and hope for patients with cancer. Oncologist,
Naturopaths, Acupuncturists, Mind Body Medicine Therapists, Nurses and support
staff are committed to delivering compassionate, integrative cancer care for the
body, mind and spirit of patients.

We are currently seeking a Patient Navigator to join our team. We are currently in
Capital Hill but will be relocating to Renton (I-5 & 405) by the end of the year.

JOB SUMMARY:
The Patient Navigator serves in the capacity of “Patient Care Concierge”. The Patient
Navigator is a point of contact responsible for orienting patients to the integrated
care model of Seattle Cancer Treatment and Wellness Center (SCTWC) and
contributes to creating and maintaining patient flow on a daily basis. This position
provides supportive consultation and coordinates services for patients throughout
their treatment. Additionally, the Patient Navigator provides post-treatment
communication and follow-up for patient and/or family members. This position
participates in promoting and facilitating the delivery of psychosocial services to
patients. The Patient Navigator demonstrates SCTWC’s Mission, Vision, Values and
patient focus to every SCTWC patient.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
 Serves as care facilitator/patient advocate to ensure Continuity of Care.
 Introduces patients to all aspects of the integrated healthcare model available at
   SCTWC.
 Assists in identification and provision of psychosocial services through direct
   provision or referring for appropriate services.
 Assists the Patient Financial Coordinator in assessing patient’s financial status,
   financial barriers and referral to resources for financial assistance.
 Communicates daily with healthcare providers, nurses, medical assistants, Mind
   Body Medicine (care team) to ensure patient direct care needs are met.
 Communicates daily with scheduling personnel to identify and assist in mitigating
   patient’s appointment scheduling, confirmation, or compliance issues.
 Facilitates on-going communication with patients prior to, during and after
   treatment with SCTWC.
 Duties and responsibilities are not all inclusive and may be expanded or changed
   as deemed necessary by the Practice Administrator.

REQUIRED SKILLS & EXPERIENCE:
 Bachelor’s Degree;
 Experience in a healthcare setting;
 Strong customer service skills.

PREFERRED SKILLS & EXPERIENCE:
 Knowledge of medical terminology;
 Oncology experience;
 Familiarity with a variety of healthcare field concepts, practices and procedures.

ADDITIONAL QUALIFICATIONS:
   Ability to provide compassionate and superior patient care;
   Excellent verbal communication and interpersonal skills;
   Demonstrated ability to independently attend to details, prioritize and complete
    multiple tasks in a high paced rapidly changing environment.
