   When registering to do business with the State it is strongly
    recommended to use a Federal Employer Identification Number

        One can be obtained at no cost through the Internal Revenue
         Service (IRS)

        Using an FEIN will minimize the risks to personal financial
         information associated with the use of a social security number

   If you do not have a FEIN number you can obtain one at www.IRS.gov

    Revenue (DOR)

    industry activities

        Full name, social security number, home address and telephone number, and
         driver license number of the business owner

    Division of Incorporation

   Failure to file an annual report will result in

        Administrative dissolution of the corporation and

        Revocation of authorization to transact business as a corporation in
         Florida

   Who Needs Coverage:

        Construction Industry: An employer in the construction industry who

        Non-Construction Industry: An employer in the non-construction industry,
         who employs four or more part or full-time employees

        Agricultural Industry: Agricultural employers with five or more regular
         employees and/or 12 or more seasonal employees, who work for more than
