ECR DESIGNS EVENT INVITATION FORM







 What is your name?





 What is the full name of the guest of honor? How are you related to this person?





 Who is hosting the event? (you, guest of honor, parents of guest of honor, etc).





 What kind of event is this? (Birthday/Bar or Bat Mitzvah/Anniversary/Baby

Shower/Bridal Shower/Communion/etc).





 What is the easiest way to get in touch with you? Email or Phone? and what is your

phone number if this is easier.





 What kind of budget are you working with for your invitations? ( what is the ideal

amount you would like to spend? what is the maximum?)





 How many invitations do you think you will need? (if you dont know definitely that is

fine! I just need an approximate number)





 What is the date of the event?





 Where is this event being held?





 Do you know the timing of the event yet?





 What colors are you planning to mostly use? (if you havent decided but just have some

ideas thats fine as well!)

 Are you looking to have any specific designs on the invitation? Are you opposed to

having any designs? What type of graphics would you be interested in? (Monogram /

Floral / Scroll / Leaves / etc)





 Are you interested in any embellishments? Rhinestones/ Ribbons/ Embossing/ Initial or

theme Charms/ lace/ bellybands/ glitter/ monograms etc..... (if there is anything you are

not sure about with graphics or embellishments etc that is fine, leave it up to me! I will

show you options and if there is anything you dont love we can change it and do

something different).





 Are you interested in a specific style of invitation? (folded, z-fold, layered

card, envelefold, pocketfold, box invitation- round or square, pillow invitations,

invitation pad under invitation, etc) I can give you a full list of options.





 What size are you interested in for your invitations or are you flexible? (5x7, 6x6 square,

6 inch round, 6x9 etc)





 What type of inserts will you need or want?

(Directions/accomodations/reception/response/Adults Only/ etc)





 Are you interested in having us address items for you? (Response cards with Names and

Number of guests, Response return addresses, Invitiations addressed, etc)





 Have you found any inspiration boards that really encapture your theme? If you have not,

would you like us to send you some or are you leaving that up to us with this information

you have provided?







 Do you feel that you may be interested in other services of ECR Designs as well?





 Is there anything else you feel we should know with regards to your event?
