Registration Form Instructions
The SSI Registration Form contains (7) Worksheet Tabs -- Instruction, CLUB Info, Club REG Totals, GIRLS Reg, BOYS
Reg, Age Groups-MIN & Age Chart
The Form is compatible with the 97-2003 version of Excel. Please keep in the same format/version.
All forms should be filled out (see all tabs on bottom) and then sent back to SSI via e-mail
(ssinterleague@optonline.net)
Final Registration Deadline to be in SSI: Saturday January 28, 2012
NOTE: Once Clubs are finished with the Spring '12 team registration, they can send in their forms and fee for the spring

SSI fee Per season is $40.00 per team.
All prior season fines must be paid to register your teams for the upcoming season.
US Club player / coach insurance is
Players per year ( Aug to July) = $10.00
Coach per every two years = $15.00
This is paid to US Club Soccer through their website(www.usclubsoccer.org)

We need three different contact persons, one each for President, Interleague Coordinator and Director of Referees.
Club REG Totals TAB --
List the total number of Boys & Girls teams for each age group division. Form will calculate totals
Totals listed on this form must match the number of teams listed on the GIRLS/BOYS Reg Forms
All teams should have the required minimums - consult Age Group MIN Tab for guidance
If the team has more than one older player on it the team MUST play up -- consult Age Chart Tab for guidance
If you are placing a team in a division above their DOB Division then put a star next to it, so we know they are playing up.
GIRLS/BOYS Reg TABs --
Girls/Boys registration forms must be filled out correctly, this is were the info is copied from for the schedules
Division: Gender then two digits ; Example Age Div : GU13 ; BU09 (Choose from Pull-down List)
If you are entering a development/tournament/(pre-travel) team in the U9 Div, MUST have the following (GU09T or BU09T)
Minor or Major (Pull-down List) & last season record should be included. Any request must be listed (ex AM/PM games),
Team Name; Club name then Div then Team name; Example -- (Elwood BU12 Tigers)
All teams must have two coaches (coach + asst coach) listed with information to contact them: e-mail & phone numbers
Additional Registration Requirements
Letters of request: On your club letter head signed by Pres or VP or Registrar with the request /only will be honored
All coaches and players must have their US club passes ( in US club data base)
Coaches without a pass or US Club Risk Management will not be able to coach / or team will be put on probation.
All coaches must have sent in their SSI Acknowledgement of Insurance/Sportsmanship each year.

US Club registration fee for Blue and Green is $ 10.00 per player for the year with the pass
US Club registration fee for Yellow is $ 25.00 per player for the year
US Club and SSI require All Adult supervisors working with the teams have to have a pass and RISK Management.
NOTE:
Any player playing up in the Adult league has to pay the US Club adult insurance fee of $ 25.00
All players on a team have to be the same insurance no mix and match (youth and adult)
