Background Check and Fingerprint Card (Return to Margaret Robinson)

1.      Criminal Background Check Form
         Complete all sections of this form. Do not leave any section blank.
         Bring the completed form to the Office of Student Affairs at the College.
         Return to College of Nursing Student Affairs to have notarized.

2.      Fingerprint Card
         You may request a card online at: http://www.bon.state.nm.us/fingerprint_form.php
         Two fingerprint cards must be submitted
         Fill out all boxes on the application, do not leave any blanks.
         You may have these completed at your local Sheriff's Department. Check the Government pages in
            your telephone book for locations and phone numbers.
         Fingerprint Certificate form must be submitted with your fingerprint card.
         Return to the College of Nursing Student Affairs to have notarized.

3.      Fee
         New Mexico Board of Nursing only accepts Cashier’s Check, Money Order or Demand Draft
         $40.25 made out to New Mexico Department of Public Safety


Bring items #1-3 above to the Office of Student Affairs at the College. They will notarize the Form and send all
materials to the New Mexico Department of Public Safety.

All parts of the application must be filed in the complete legal name omitting ALL references to nicknames, etc.
Do not indicate anticipated name changes. Print legibly and in black ink. Signatures must be in black ink.

1. Applications are revised periodically so DO NOT request an application unless you intend on completing it
   within a 30 day period.

2. All sections must be completed in full. If an item is not applicable, indicate with N/A.

3. Only a nurse’s LEGAL name will be used for licensure in New Mexico. Name changes require a copy of filed
   legal documents sent to the New Mexico Board of Nursing.

4. The application fee is $110 and is non-refundable.

7. Program Completion: May 2009

8. The Certification of Eligibility for Graduation of Nursing Program must be sent to the New Mexico Board of
   Nursing directly from the Registrar’s office. Return this form to the College of Nursing Student Affairs; they
   will have the registrar sign and will send to the NM Board.



The Office of Student Affairs will then Fed-Ex your completed application and transcripts to the NM Board of
Nursing following graduation. It may be a few weeks following graduation before you receive your Authorization
to Test letter.




STEP 2: NCLEX APPLICATION (must be completed by mid May, 2009)
Read the NCLEX Candidate Bulletin instructions thoroughly before completing your registration.
There are three options on how to register for NCLEX: 1) Registering on-line at www.vue.com/nclex, 2) Registering by
telephone, and 3) Registering by mail. The College recommends option #1.
1.   Go to www.vue.com/nclex. Select the "REGISTER" link on the right hand side.
2.   Enter your complete name as it will appear on the photo identification you will use when you take the exam. This will
     need to be the same name you put on your Board application.
3.   Enter a non-OUHSC e-mail account. Your OUHSC account many not be active when your ATT is released and as such
     you should use an alternate address. Your ATT letter will be sent to this e-mail address.
4.   The College encourages you to answer all questions. Your submission of a SSN and previously used names will confirm
     your identity and prevent inconsistencies between your Board application and NCLEX registration.
5.   You are registering for the NCLEX-RN exam, in the United States.
6.   Select the school (campus) that you attend:
     Oklahoma City:        "24-552, United States, OK, OKLAHOMA CITY, UNIVERSITY OF OKLAHOMA-BS"
     Tulsa:                "24-527, United States, OK, TULSA, UNIVERSITY OF OKLAHOMA – BS"
7.   Enter the following graduation term: 05/2009
8.   Select the following Board of Nursing: "New Mexico Board of Nursing."
9.   Enter your credit card information and complete your registration. The charge is $200 and due at time of registration.
     Print any confirmations or e-mails and keep them for your records.
                                   REGISTRATION ≠ AUTHORIZATION TO TEST
     Your ATT letter will come 2-4 weeks after your degree is posted. Only then can you schedule your date to test.
