The Samouel’s Employee Questionnaire
Screening and Rapport Questions
Hello. My name is ____ and I work for DSS Research. As you know, Phil Samouel has hired my
company to conduct a survey of its employees to better understand the work environment, and suggest
improvements as needed. The survey will only take a few minutes and it will be very helpful to
management in ensuring the work environment meets both employee and company needs. All of your
answers will be kept strictly confidential. In fact, once the questionnaires are completed they will be taken
to my office and kept there.

If they are currently employed by Samouel’s restaurant and do not have any questions, hand them the
survey and ask them to complete it. Tell them to ask you if there is any thing they do not understand.
