Receive a Free Report on Customer Satisfaction Surveys
PIA/GATF is currently developing a publication of sample customer satisfaction surveys in use today in
graphic arts firms. The intent of this project is to collect a variety of “home-grown” customer
satisfaction surveys and publish in a book later this year.
The surveys can come from any part of the industry. For example: large or small firms; commercial,
packaging, publications, or in-plant firms; pre-press, press, bindery, mailing, or fulfillment firms; or
even suppliers to the industry.
To develop this publication, we need YOUR help. First, by completing our attached survey and second
by supplying the customer satisfaction survey your firm uses. PLEASE NOTE: We cannot use surveys
from third-party vendors or ones that have a copyright.
If you complete the form on the back AND send us at least one customer satisfaction survey that your
company uses, you will receive a free copy of the final publication.
Not all surveys received will be used in the final publication. All documents used will have company
