value from that facility? Use this item bank to draft a brief survey tailored to your own needs. Just use a
few of these questions, rewrite them if you need to, and add questions of your own if you like.
Most of these questions are framed in a positive way (e.g., Agree/disagree: "The total amount of text is
satisfactory") but you may find you get more pointed, useful feedback when you mix in questions about
potential problems (e.g., Agree/disagree: " There is so much text on the screen that it is hard to read them.")
