We are looking for care workers to cover the Redbridge area (Ilford). Carers will be required to visit

Job Title: Health Care Assistant
Band: 2
Hours: 37.5 hours per week

Primary Care Trust reserves the right to require you to work at any of its establishments on a
permanent or temporary basis.
Accountable to: Community Nurse Manager
Responsible to: District Nurse / Community Matron
Job Purpose: To provide holistic care. The Health Care Assistant works under the
supervision of the District Nurse /
Community Matron as part of the Primary Health
Care Team. Holistic care is provided for patients in
a variety of community settings.
Main Duties and Responsibilities:
1.0 Clinical Duties:
1.1 To give holistic care to patients in the community, such as in the patient’s home, health
centre / clinic, residential home and GP surgery.
1.2 To identify and report any changes in patient care needs or changes in physical or
emotional well – being to the District Nurse. To report any changes in family / carer
circumstances.
1.3 To maintain accurate records in the patients own home or elsewhere, record new
problems as they are identified and report all actions taken to the District Nurse.
1.4 To act as the patient’s advocate and, by providing information and support, facilitate the
patient’s own choices with regard to personal care, promoting independence and self – care
as appropriate.
1.5 To be skilled in communication, establishing and maintaining good relationships with
patients, carer, and members of the Primary
Health Care Team and other agencies involved, making referrals where appropriate to ensure
adequate support to patients and carers and reporting these actions to the District Nurse.
1.6 To offer support and to teach patients, relative and other carer’s appropriate skill,
imparting knowledge to promote positive health.
1.7 To have knowledge of and able to effectively use available resources.
1.8 To participate in teaching and support of learners in the locality.
2.0 Administrative Duties:
2.1 To maintain accurate records and statistical returns as required by the Trust, including
entering data onto the RIO system within the required time frame.
2.2 To attend and contribute to team, locality and other relevant meetings.
2.3 To report and document any incident or accident to a patient or member of staff.
2.4 To be responsible for the care and safe keeping of equipment issued and to report any
defect or loss.
2.5 To carry out delegated tasks as necessary to support the work of the team as instructed
by the District Nurse and other team members.
3.0 Personal and Professional
3.1 To be aware of and act in accordance with PCT Operational Policies.
3.2 To be aware of and act in accordance with the Health and Safety at Work Policy.
3.3 To participate in Clinical Supervision on a regular basis with a designated person

Prevention and Control of Infection
The PCT has prioritized the prevention and control of infection and the full implementation of
the Health Act (2006) as a core component in the organisation’s clinical governance and
patient and staff safety arrangements. Infection prevention and control is the responsibility of
every member of staff. In consequence, all employees are expected to:-
i) Follow consistently high standards of infection control practice, especially with reference to
hand hygiene including hand washing, adherence to dress/ uniform code, and for clinical staff
adherence to all clinical policies
ii) Keep up to date and comply with all PCT’s infection control guidelines, procedures and
policies relevant to their work.
iii) Participate in induction, mandatory training and annual updates
iv) Clinical staff will seek support and advice from the infection control nurse/practitioner when
managing situations involving patients with infections where guidance provided is not
available/appropriate or in instances where cross infection is likely to have occurred.
v) All staff will contact occupational health if they are suffering from any form of infection,
which may put patients and other staff at risk.
