Cancellation Policy
We are asking that you extend the courtesy of adequate time to reschedule
valuable appointment time that was reserved just for you. If you know that there is a
problem with your appointment time, the sooner you contact my office administrator, the
more effective she can be in allowing another patient the opportunity to fulfill their
treatment needs. We ask that you give us 24-hour notice on appointments that are
scheduled for a one-hour duration or less, and an additional 24-hours per 30 minutes of
appointed time for appointments scheduled in excess of one hour. For example, an
appointment scheduled for three hours would require a one week notice. For individuals
that do not comply with this request, a fee of at least 25% will be assessed for planned
treatment. For individuals who cancel appointments within the 24-hour period, you will
be charged for the appointment scheduled. Chronic abusers of scheduled appointments
will be dismissed from the practice.

smoothly and efficiently for you and for the rest of our patients.
