                                                      • We chose these goals
                                                        because staff buy-in

              Data Indicators
• We collect attendance
  data monthly from our
  secretary and track
  positive behavior using
  our “Caught Being
  Good” slips and ODRs /
  “White Slip” forms,
  which were redesigned
  this year to match the
  positives.

              Implementation
• “Caught Being Good” slips were re-designed and “White Slips”
were changed to include current UBI goals.
• The staff was asked what changes they wanted. We listened
and implemented many of their suggestions, such as non-
tangible rewards for winning on the 100 club board, re-
designing the white slip (four times…), and changing the staff
  reward.
• The staff wanted to have faculty lunches for their rewards this
year, so we held one per quarter.
• Traditional sources of data were utilized for the attendance and
  tardies.
• We customized our data sources by making changes to our
“Caught Being Good” slips and creating a 100 board.

           Plans for
    Continuous Improvement
• We plan to continue using our flip chart as
problems come up and for staff training
  purposes.
• We hope to find a way to track minor
  offenses.
• We may need to re-evaluate our detention
system and find ways to make it more
  effective next year.
