CONSENT FORM FOR USE OF PHOTOGRAPHIC / VIDEO IMAGES
During the year, Teatru Manoel may occasionally need to take photographs or video
recordings of you or your child to appear on our website, printed publications, and other
material for Toi Toi promotion.
As you may be aware in order to comply with the Data Protection Act 2001, we need your
permission before we take or use any images of you or your child.
Please answer questions 1, 2, 3 and 4 below, then kindly sign and date the form where
shown and return to Teatru Manoel.
Kindly note that your child will not be included in any filming / photo session should this form
not be returned.

The image will be unnamed and as far as possible copy-protected.

Please also note that irrespective of whether you consent or not to the above 4 requests:
We will not include details or full names of any child or adult in an image on our website, or
in printed publications without consent.
We will not include personal e-mail or postal addresses, or telephone or fax numbers on
our website or in printed publication without consent.
If we use images of individual children or adults on the web, we will not use the name of
that individual in the accompanying text or photo caption. If an individual is named in the
text, we will not use a photograph of that person to accompany the article.
We may use group images with very general labels.
We will only use images of individuals who are suitably dressed.
