Anglers’ Challenge Entry Form
• A current MWA membership is required for each angler participating in any MWA event.
• All events will run from 7am-3pm EST with a mandatory Captains’ meeting prior to each event at 6:30am.
• Event entry fees are per boat, with “boat” defined as either (1) or (2) participants; meaning “Captain” or “Captain & Angler”.
• The deadline for event entry forms & full payment is 7pm Tuesday the week of the desired event. Any entry forms or payment
received after the deadline will be deemed “late” and incur a late entry fee. Late entry forms will be accepted until the start of
the Captains’ meeting the day of the event & in the order received. Should we exceed the 25 boat maximum field size, Cabela’s
NTC participants whose entry fees are not late, will be accepted before non-Cabela’s NTC participants.
• All fees, memberships, & registrations are non refundable per the 2012 Anglers’ Challenge Official Rules & The Event
Director & Committee reserves the right to accept or reject any entry, for any MWA Event, for any reason.
• Event takeoff positions will be drawn at each Captains’ along with a voluntary big fish pot of $20 per boat (except classic).
• A one time registration fee is required to participate in the Cabelas NTC Points Program with the MWA. Each registering team
will be provided with individual memberships in The Walleye Federation if not already a member.

(There will be a voluntary rules meeting for all interested teams on Wed. April 18th @ 6:30pm to discuss rules, procedures, & to answer any questions)

. fished as selected by each Committee Member & Director
Liability, Photo, & Video Releases
In signing this agreement, I (we) hereby release the Michiana Walleye Association, Inc., its officers, employees, sponsors & their agents
(hereinafter referred to as MWA) from any & all damages, claims, demands, costs or expenses relating to injuries, death, damage, theft,
fire or any loss of any persons or any property which I (we) may sustain and/or which I (we) may cause by reason of participating in or
in connection with these events. I (we) further release the MWA, for all damages relating to adverse weather while participating in said
events. It’s my responsibility to seek safety should adverse weather conditions occur (prior to, during, or after the event). I (we) signify
by my signature that I (we) received, reviewed, read and agree with the rules & releases of the 2011 Anglers’ Challenge Official Rules. I
(we) understand & agree all participating boats must have the required Liability Insurance. I (we) also acknowledge that these events do
not guarantee, but rather may, result in an invitation to the Cabela’s National Team Championship next year. Neither the MWA nor
Cabela’s will guarantee this outcome as these events are considered among other nationwide events all vying for the same result. I (we)
agree that photos, film, or video taken during the event may be used for commercial purposes by sponsors, event organizers, or news
media without royalties or prior consent, unless specified in writing prior to said events. No unsigned entry will be accepted for events.
