disasters, invites you to take part in a healthful three-mile (5K) noncompetitive walk/run

WAIVER AND RELEASE AGREEMENT
Shaklee Fun Walk/Run in Anaheim, CA (“Event”)
I, the undersigned, am an independent distributor of Shaklee Corporation (“Shaklee Distributor”) or am a family
member of a Shaklee Distributor. I have voluntarily chosen to participate in the Event sponsored by Shaklee Corporation
and understand that my execution of this Waiver and Release is a prerequisite for participation.
I, the undersigned, understand that my participation is not part of my work as a Shaklee Distributor, is not a
reasonable expectation of my independent distributorship and is neither expressed nor implied required by Shaklee
Corporation (or its related entities).
I, the undersigned, acknowledge that I am physically capable of safely participating in and completing this Event,
and if I am a parent or guardian signing on behalf of a person who is less than 18 years of age, I acknowledge that I am
strongly encouraged to personally accompany and provide adult supervision for such person during the entire Event.
I, on behalf of myself, my heirs or successors in interest, hereby assume all risks associated with participation
in the Event in Anaheim and I release and agree to indemnify and hold harmless Shaklee Corporation (and its related
entities), and its respective officers, directors, employees, agents and representatives, the City of Anaheim, the Anaheim
Police Department, Off ‘N Running Tours, their agents, employees and related companies from all actions, claims,
liability, demand, damages, costs and expenses (including reasonable attorney’s fees) arising out of, any and all known or
unknown damages, including but not limited to personal injury, property damage, and any other damages arising directly
or indirectly from the undersigned’s participation in the Event in Anaheim.
This agreement is governed by the laws of the state of California.
I ACKNOWLEDGE THAT I HAVE READ AND THAT I UNDERSTAND EACH AND EVERY ONE OF THE PROVISIONS IN
THIS WAIVER AND RELEASE AND ACKNOWLEDGE THAT THEY SHALL BE EFFECTIVE AND LEGALLY BINDING.
If I am under 18 years of age at the time of registration, my parent or legal guardian has completely reviewed this Waiver and Release and
understands and consents to its terms, and authorizes my participation by his/her signature below.
