Education Tax Refund If your family is entitled to ‘Family Tax Benefit A’ a 50% rebate is available for costs including education software,
home computers, internet connection costs etc. The maximum rebate is $397 for each child in primary school &
$794 for each child in secondary school.
Medical Expenses Net medical costs over $2,000 ie after Medicare and Private Health Insurance Reimbursements (excluding private
health insurance premiums)
HELP/SFSS Debt If you have HELP debt or SFSS debt please provide details
Spouse Adjustable Taxable If we do not prepare your spouse’s income tax return please advise their 2011 adjusted taxable income.
Children Please provide names and dates of birth of any children born during the year.
Flood Levy Are you exempt from the Flood Levy due to suffering from a Natural Disaster that occurred in 2010/11? (This is
relevant in calculating PAYG-Instalment tax for the 2011/12 financial year). If you are unsure please contact us.
