Fundraiser Raffle Forms
This document contains templates for the following 3 types of
raffle tickets:
1. A raffle open to people of all ages, for one prize.
2. A raffle open only to adults
3. A raffle where there are multiple prizes, where it is desired
that an entrant specify the prize being sought.
ALL INFORMATION AND FORMS ARE PROVIDED “AS IS” WITHOUT ANY WARRANTY,

[Note: These templates have random 4-digit numbers. In a one-time raffle, these numbers are optional.
If such numbers are to be used for extra security, of course those numbers should be consecutive. The
secretary should keep track of the ending number for a particular raffle, to ensure that only "fresh"
tickets can be used at a subsequent raffle.]
