CARLISLE AREA HEALTH & WELLNESS FOUNDATION
MINI-GRANT APPLICATION FOR SCHOOLS
Mini-Grants for school-related activities are open to every school building in the Foundation
geographical area. These mini-grants will be available to provide financial support for schools to
enhance health and physical education opportunities beyond standard educational requirements.
CAHWF works closely with the state-wide nrg Balance program (formerly PANA) and we encourage
schools to enroll as an nrgBalance Zone. Please check out www.nrgBalance.org for more
information.
CAHWF will continue to fund some of the usual requests for playground equipment and health fairs;
however, as competition increases for grant funding, we look favorably to innovative ideas such as:
- Adding non-competitive intramural sports
- Offering after school activities that increase youth exercise
- Improving health curricula or other opportunities to teach good nutrition
- Promoting policies and environments that support healthy eating and activity.
Please check our website for general guidelines to our grant making process. All CAHWF
guidelines apply to the School Mini-Grants in addition to the following:
Proposals for Mini-Grants may be submitted at any time.
Decisions on Mini-Grants will generally be made within 30 days.
Mini-Grant awards range from $200 - $2,000.
Each elementary, middle or high school building within CAHWF’s geographical area is
eligible to apply.
Schools may apply for multiple Mini-Grants, but the total amount awarded is not to exceed
$2,000 per fiscal year per school building. (July 1 to June 30.)
The Building Principal is a required signature on all school applications.
Foundation resources are limited, as with other grant levels, and funding is not guaranteed.
CAHWF accepts only complete proposals using the format outlined below and the attached
Information Sheet. Please submit one original and 5 copies to:

PROGRAM TITLE:
1. Describe your project, the need for it and anticipated numbers served. Be very clear about the
activities that will occur.
2. How will you determine whether your objectives have been achieved and whether your project is
successful?
3. What is your projected timetable and who are the key people involved?
4. Provide a project budget.
