Payments in Full are required on each MONDAY of each Week by 9:00 am. You will be
charged a $5.00 late fee for each day you are late and if your payment is received after 9:00 am
on Monday.
My weekly payment is:
$_______________________per week and/or $________________per hour, per child; even if my
child/ren do not attend regardless of the reason; vacation, illness, etc. This includes center closed
holidays. I am still responsible for the weekly full payment/s.
I understand I will be charged an additional $4.00 per hour for each non-school day/s, non-contracted
hour/s and/or early out/s; IF SPACE IS AVAILABLE.
*Overtime Fee: I agree to be charged a $15.00 per 15 minutes or any increment of 15 minutes when
time wasn't prearranged with owner. Late arrival does not justify late departure. Bullfrogs, Bugs and
Butterflies is under no obligation to provide an extension of time. If the parent/s/guardian will be
arriving earlier or later than the agreed upon times as written in this contract, the overtime rate/s will
apply.
*****Every 2 weeks contracted hours will be reviewed, if contracted hours are being abused my weekly
contacted payment will change with a written notice from the owner. Ex: Contracted for 30 hours of
care but is using 37 hours of care each week.
Initial Enrollment is on a two week, 10 day probationary status; this means if the child, the parent/s, or
over all center cannot accommodate each other; care will be immediately terminated by the owner; my
weekly payment and my enrollment fees will not be reimbursed.
This contract can be terminated by any of the Parents/Legal guardians/Co Signers after the 10 day
probationary status period with one week written notice.

If a parent or legal Guardian is under the age of 19, a co-signer must sign the contract and be bound by
all terms.
I understand my child/rens spot in the center is not secure until all the required forms and payments are
received by the center.
An enrollment fee of $30.00 per child is due upon signing this contract; along with your first weekly
payment; for the week of August 10, 2009-August 14, 2009. Both of these payments will be forfeited if
the child does not come for care as agreed.
By signing this contract, all parties agree to abide by any and all written policies of the provider/center;
this includes the center handbook. Any amendments to the policies will follow a one week notice period
before they go into effect.
