CONTRACT FOR
PSYCHOLOGICAL TESTING & MULTI-FACTORIAL EVALUATIONS
MULTI-FACTORIAL EVALUATIONS $1200 per student
This includes the standard battery of tests for learning disability, evaluation, interviews, and meetings
• Psychological Testing – WISC, WAIS, WPPSI
• BASC-2 for Parents and Adolescents
• Woodcock Johnson III or WIAT
• Conner’s Rating Scales for Parents and Teachers
• Other necessary tests for Learning Disability, as needed
• Interviews with the student, parents (or guardians), and teachers
• MFE Report and updates within 6 months
• Meetings with Parents (or Guardians) for feedback of evaluation
• Meetings with Parents (or Guardians), Staff, and other officials necessary for the IEP
ADDITIONAL SERVICES
Additional services that may be needed (outside of Learning Disability) such as Projective Drawings (and
report) OR psychological diagnosis (and report) is an additional $300 per student.
BILLING
Our company sends out invoices at the end of every month. Terms are 3% discount if paid within
30 days of invoice. Late fee charge of 3% after 60 days of invoice.

This contact may be cancelled at any time by the Client. In case of a cancellation, please allow 30 days
for final delivery of MFEs in progress. The Client will be notified 30 days before any changes in the
Contract will take effect.
