*Programs covered by this agreement must be completed within two (2) years of enrollment.
The Event Planning Institute charges a $100 enrollment fee to be submitted with your enrollment form. In addition, a $200 confirmation
fee must be paid within 10 business days of enrollment acceptance, to secure a space in the program. This $200 confirmation fee is
credited towards the total cost of books and materials.

CANCELLATION AND REFUND POLICY:
Rejection: An applicant rejected by the school is entitled to a refund of all monies paid.
Three-Day Cancellation: An applicant who provides written notice of cancellation within three (3) days of signing an enrollment
agreement, excluding Saturday, Sunday and state and federal holidays is entitled to a refund of all monies paid. No later than 30
days of receiving the notice of cancellation, the school shall provide the 100% refund.
Other Cancellations: An applicant requesting cancellation more than three days after signing an enrollment agreement and making
an initial payment, but prior to entering the school, is entitled to a refund of all monies paid minus a cancellation fee of $200.
Refund after the commencement of classes:
1. Procedure for withdrawal/withdrawal date:
     A. A student choosing to withdraw from the school after the commencement of classes is to provide written notice to the
           Director of Student Services. The notice is to indicate the expected last date of attendance and be signed and dated by the
           student.
     B. Students unable to complete the required clock hours have the option to complete these hours (with instructor approval) in
           future sessions without additional tuition.
     C. For a student who is on authorized Leave of Absence (LOA), the withdraw date is the date the student was scheduled to
           return from the leave and failed to do so.
     D. All refunds will be issued within 30 days of the determination of the withdrawal date. Tuition refunds will be issued by check
           only, with a 3% processing fee deducted if original payment was made by credit card.
2. Tuition charges:
     Tuition charges will be determined based upon the percentage of the clock hours attempted. The percentage of the clock hours
     attempted is determined by dividing the total number of clock hours elapsed from the student’s start date to the student’s last day
     of attendance, by the total number of clock hours in the program (less the $200 cancellation fee).
     Tuition refunds will be issued within 30 days of the date of student notification, of date of school determination (withdrawn due to
     absences of other criteria as specified in the school catalog), or in the case of a student not returning from an authorized Leave
     of Absence (LOA), within 30 days of the date the student was scheduled to return from the LOA and did not return.
     Approved tuition refunds are determined as follows:
     1. Before the beginning of classes, the student is entitled to a refund of 100% of the tuition less the $200 cancellation fee.
     2. After the commencement of classes, the tuition refund amount shall be determined as follows (less the $200 cancellation
           fee):

    3.   Tuition refunds will be issued by check only, with a 3% processing fee deducted if original payment was made by credit card.
    4.   Books and Supplies/Workshop fees: There is no refund for equipment, books and supplies received by the student. Any
         workshops paid for and attended by the student are not refundable. The toolkit consists of the basic equipment, texts and
         materials required for beginning each program. A list of the components of the toolkit is provided to each enrolled student.
         Students who leave the program during the first day of attendance may return materials, and/or individual components of the
         toolkit. The student may receive a refund or credit, if the item(s) being returned is in good condition and is resalable.
            Assignments: In addition to regular attendance at scheduled classes, or online classes, the student will be required to
            devote additional time outside the classroom studying, testing, and working on assigned projects. All students must have
            access to an IBM® compatible PC with Internet and email access.
            Curriculum: The Event Planning Institute reserves the right to revise course contents, course titles and the sequencing of
            classes, subject to applicable regulatory approval.
    5.   Special Cases: In case of prolonged illness or accident, death in the family, or other circumstanced that make it impractical
         for the student to complete the program, the school may make a settlement that is reasonable and fair.
    Holder in Due Course Statement: Any holder of this consumer credit contract is subject to all claims and defenses which the
    debtor could assert against the seller of goods or services obtained pursuant hereto or with proceeds, hereof. Recovery hereunder
    by the debtor shall not exceed amounts paid by the debtor.

THE STUDENT UNDERSTANDS:
     1.      The school does not accept credit for previous education, training, work experience (experimental learning), or CLEP.
     2.      Upon successful completion of the program, the school will assist any graduate requesting job placement; however, the school
             does not guarantee employment to graduates upon program/course completion or upon graduation. A certificate will be issued to
             each student who successfully completes the program and satisfies all requirements.
     3.      The school reserves the right to reschedule the program start date when the number of students scheduled is ten (10) or less.
     4.      The school will not be responsible for any statement of policy or procedure that does not appear in the school catalog.
     5.      The school reserves the right to discontinue the student’s training for unsatisfactory progress, nonpayment of tuition or failure to
             abide by school rules.
     6.      The school does not guarantee the transferability of credit to a college, university or institution. Any decision on the comparability,
             appropriateness and applicability of credits and whether they should be accepted is the decision of the receiving institution.
     7.      The student is responsible for tuition and fees pertaining to the program’s required course of study. The tuition and fees
             contained in this Enrollment Agreement are subject to change. Any changes to the tuition and fees will be published to students
             as they are made.
     8.      The Event Planning Institute does not discriminate on the basis of race, color, creed, religion, national origin, ancestry, sex, age,
             sexual orientation, or disability or any other characteristic protected by state, local or federal law in the administration of any of its
             educational programs or activities, or with respect to admission or employment. For information on the Event Planning Institute’s

GENERAL INFORMATION & UNDERSTANDING
In the event of a dispute arising out of this Agreement, Event Planning Institute shall be entitled to reimbursement of all reasonable
attorney's fees and costs expended by the institute in the resolution of the dispute or collection of monies due; and in the event of
litigation, the prevailing party shall be entitled to recover reasonable attorney's fees and costs. The Event Planning Institute cannot be
held liable if the student is unable to obtain certification or licensing due to information not disclosed. If the student complaint cannot be
resolved after exhausting the Institution’s grievance procedure,* the student may file a complaint with the Arizona State Board of Private
Post-Secondary Education. The student must contact the Arizona State Board for Private Postsecondary Education for further details.

  payment of any collection fees or attorney fees incurred by the Event Planning Institute.

TUITION PAYMENTS:
1.   Tuition deposit of $100 is due with signing of the enrollment agreement. A books & materials fee of $200 is due ten (10) business days of
     enrollment acceptance.
2.   Tuition options:
     A.       Payment Option 1: FULL PAYMENT PLAN
              I agree to pay the full program amount, and receive a tuition discount of $100. The program cost after discount is $1700.
              $25 shipping charge for books and materials is NOT assessed with this payment option.
     B.        Payment Option 2: AUTOMATIC PAYMENT PLAN
               I agree to pay the $100 Enrollment Fee now, and have the $200 books & materials fee automatically charged to my credit card ten
               (10) business days upon enrollment acceptance to secure a space in the program. The remaining balance of $1500 is to be
               automatically charged in three (3) equal payments of $500 with the total program cost of $1800 paid in full 2-weeks prior to the first
               day of scheduled program.
               Home-Study students will be assessed an additional $25 shipping fee for books and materials, with the total program costs totaling
               $1825.00.
     C.        Payment Option 3: AUTOMATIC PAYMENT PLAN
               I agree to pay the $100 Registration Fee now, and have the $200 books & materials fee automatically charged to my credit card ten
               (10) business days upon enrollment acceptance to secure a space in the program. The remaining balance of $1500 is to be
               automatically charged in six (6) equal payments of $250 with the total program costs of $1800 paid in full 2-weeks prior to the first
               day of scheduled program.
               Home-Study students will be assessed an additional $25 shipping fee for books and materials, with the total program costs totaling
               $1825.00.

3.   These payment options are available to all students.
4.   Fees. Required books and administration costs are included in the $300 fee.
