To receive your mail-in rebate:
— Purchase any Tenba Black Label bag from an Authorized Tenba Dealer in the USA between 7/1/2009
and 8/31/2009, for immediate delivery. Limited to one rebate per customer. Cannot be combined with
any other discounts or coupon codes.
— Send in this completed form, along with a copy of your cash register receipt and the original UPC
label(s) from the back of the product hangtag(s).

— Cash your rebate within 90 days of the check date.
