1. PROJECT DESCRIPTION AND OVERVIEW: Demolition, renovation, and new construction as
indicated on drawings and specifications to relocate existing ICU to Building 100, 4 th floor East at the
VAMC, Columbia, SC.
1.1 The contract will be Firm-Fixed Price (FFP). For the evaluation of proposals, the government has
developed a Source Selection Plan. The purpose of the Source Selection Plan is to establish a uniform
evaluation procedure for the technical evaluation of proposals by the Source Selection Evaluation Board
(SSEB) and the development of the Best Value Decision by the Source Selection Authority (SSA) using the
Trade-Off Process (See Federal Acquisition Regulation 15.101-1). In as much as the proposal shall
describe the capability of the Offeror to perform any resulting contract, the proposal should be specific and
complete in every detail. The proposal should be prepared simply and economically, providing a
straightforward and concise description of capabilities to satisfactorily perform the contract. The proposal
should be practical, legible, clear, and coherent.
1.2 Proposal Submissions and the Best Value Tradeoff Process. This process permits tradeoffs among cost
or price and non-cost factors and allows the Government to accept other than the lowest priced proposal.
Offerors submit their Corporate Relevant Specialized Experience and past performance information for
review and consideration by the Government. Following the review, evaluation, and rating of these
proposals, the Government will evaluate price proposals for contractors that have acceptable technical
proposals. The technical information contained in the proposals shall be reviewed and evaluated by the
Government in accordance with the evaluation criteria provided in paragraph 5, Evaluation and 8, Rating
Methodology. The SSEB reviews, evaluates, and rates the proposals against the source selection criteria in
the RFP. Price information will not be rated, but will be evaluated in accordance with FAR Part 15,
funding limitations set out in Section Section B – Bid Schedule, and in comparison to other Offerors’ price
proposals.
2. PROPOSAL SUBMISSION INSTRUCTIONS
2.1 Who May Submit. Any legally organized HUBZone Certified Small Business (HZ) may submit a proposal.
PROPOSALS MUST STATE CURRENT HUBZONE STATUS. The Government reserves the right to verify
that the Offeror is on the List of Qualified HUBZone Small Business Concerns prior to evaluations. If the
Offeror is not on the list, they will be considered non-responsive. A joint venture may be considered a
HUBZone small business if the business entity meets all the criteria in 13 CFR 126.616.

2.2 Where to Submit. Offerors shall submit their proposals to the Savannah District, Charleston Office at the
address shown in Block 7 of the Standard Form 1442.
2.3 Submission Deadline. Proposals shall be received no later than the time and date specified in Block 13 of
Standard Form 1442.
2.4 General Requirements.
2.4.1 In order to effectively and equitably evaluate all proposals, the Contracting Officer must receive information
containing sufficient detail to allow review and evaluation by the Government. Proposal clarity, organization, and
cross-referencing are mandatory. Failure to submit and organize proposals as requested may adversely affect an
Offeror's evaluation. Offerors should provide sufficient detail and clearly define all items required in this section.
2.4.2 Tabs. Proposal shall be organized and tabbed as shown in paragraph 2.5 Submission Format.
2.4.3 Size of Printed Matter Submissions.
2.4.3.1 Written materials shall be prepared on 8-1/2" x 11" paper.
2.4.4 Number of Copies. Offerors shall submit one (1) hard copy of Volume I and four (4) hard copies of Volume
II of their Proposal. Volume II shall also be submitted on a CD-ROM.
2.5 Submission Format.
2.5.1 The proposals shall contain a detailed table of contents. If more than one binder is used, the complete table of
contents shall be included in each. Any materials submitted but not required by the solicitation, (such as company
brochures), shall be relegated to appendices. The Proposal will be tabbed and submitted in three ring binders in the
following format:

TAB J - FACTOR 2: PAST PERFORMANCE
4.1. FACTOR 1: CORPORATE RELEVANT SPECIALIZED EXPERIENCE:
Provide examples of three projects for which the Offeror has been responsible. The examples should be as similar as
possible to this solicitation in project type and scope. Similar means Demolition and Construction projects with a
value greater than $5 Million within a hospital environment. If the Offeror represents the combining of two or more
companies for the purpose of this RFP, each company should list project examples. Example projects must have
been completed or substantially completed (at least 60% complete) within the last five years.
The examples should include at a minimum the following information:
Contact names and telephone numbers for all Project(s)
General character of the project(s)
Scope of the Project(s)
Location of the project(s)
Cost of the project(s)
Date of completion of the project(s)
Organizational Chart: The chart should identify the key personnel and individuals on the project team that show
clear lines of authority, responsibility, and communication between corporate staff, key on-site staff, and
subcontractor interface. Offerors should propose a logical flow of authority and demonstrate that assigned
responsibilities and experience of proposed on-site personnel will enhance the operation of the project.
Schedule: The Offerors shall provide a schedule that demonstrates the offeror’s knowledge of appropriate
sequencing and duration of work activities. The schedule should identify all major tasks expected during project
performance recognizing the appropriate sequence and realistic duration of each task.
4.2 FACTOR 2: PAST PERFORMANCE
4.2.1. Past Performance Information. The Offeror shall submit past performance information on three (3)
completed, or substantially completed (60% or more) projects in the past five (5) years that are relevant in scope,
complexity, and size to the work identified in the SOW.
4.2.2. Projects should demonstrate work similar to that described in the RFP. Below is a list of project information
that needs to be identified for the projects submitted.

4.2.3. If the Offeror is made up of separate construction companies that have combined for this project, then Past
Performance Information must be completed for each company.
4.2.4. Contractor Team Arrangements. Contractor Team Arrangements are considered an arrangement in which (1)
two or more companies form a partnership or joint venture to act as a potential prime contractor; or (2) a potential
prime contractor agrees with one or more other companies to have them act as its subcontractors under a specified
Government contract or acquisition program.
4.2.5 Offeror proposed teaming arrangements, as defined above, and company relationships shall be fully disclosed
in the proposal. All prospective Joint Ventures must comply with Part 13 Code of Federal Regulations (CFR)
124.513. For reference and evaluation purposes the Offeror shall identify the major or critical aspects to be
performed by those identified in the Contractor Team Arrangement.
4.2.6 Other Sources. The Government may contact sources other than those provided by the Offeror for
information with respect to past performance. These other sources may include CCASS (Construction Contractor
Appraisal Support System), telephone interviews with organizations familiar with the Offeror’s performance, and
Government personnel with personal knowledge of the Offeror’s performance capability.
5. EVALUATION FACTORS
5.1. FACTOR 1: CORPORATE RELEVANT SPECIALIZED EXPERIENCE. The Government will review
the example projects provided by the Offeror to evaluate and rate the recent experience of the Offeror in similar
projects. Similar means Demolition and Construction projects with a value greater than $5 Million within a hospital
environment. Projects should include experience in working within a phased schedule within a hospital environment.
Offerors are encouraged to provide descriptive analysis of why they feel their example projects meet the criteria for
corporate relevant specialized experience. If the Offeror cannot provide suitable relevant experience and the
evaluators consider that the information provided indicates that the Offeror has no relevant experience, a
determination will be made as to the risk this lack of corporate experience presents to the Government and the
proposal may be rated UNACCEPTABLE
5.2. FACTOR 2: OFFEROR PAST PERFORMANCE. The Government will evaluate the Offeror's past
performance using the sources available to it including the example projects identified by the Offeror, and
information available from CCASS. Offerors may be provided an opportunity to address any negative past
performance information about which the Offeror has not previously had an opportunity to respond. The
Government treats an Offeror's lack of past performance as neutral that is, having no positive or negative evaluation
significance. The Government will evaluate past performance based on the elements listed below:
Quality of Work. Based on information provided in the example projects provided by the offeror and other
information, the Government will assess the quality of the actual construction undertaken and the standards
of workmanship exhibited by the Offeror’s team.
Timeliness of Performance. The Government will evaluate all information available with respect to the
Offeror completing past projects within the scheduled completion times.

Documentation. The Government will evaluate all information available with respect to the timeliness and
quality of the documentation, reports, and other written materials completed by the Offeror on past projects.
Customer Satisfaction. The Government will evaluate all information available with respect to the
Offeror’s past customer satisfaction, cooperation with customers, and interaction on past projects.
Subcontractor Management. The Government will evaluate all information available with respect to the
Offeror’s management of subcontractors on past projects.
5.3. Contractor Team Arrangements Evaluation. In accordance with FAR Part 9, the Government will recognize the
integrity and validity of contractor team arrangements; provided, the arrangements are identified and company
relationships are fully disclosed in the offer. Past Performance evaluation will not take into account the past
performance information regarding predecessor companies; or subcontractors that will perform major or critical
aspects of the requirement, unless identified in Contactor Team Arrangements. The submission must contain a
narrative that clearly explains the relevance of the submitted information concerning a company that is part of a
Contract Team Arrangement. The Government will consider the adequacy of this explanation in deciding the
relevance and weight of the information to this procurement.
5.4. The Government treats an Offeror's lack of past performance as having no positive or negative evaluation
significance. Past performance, which is considered by the Government to be similar in size and scope to this
project, as defined in paragraph 4.1 of this section, may be considered by the Board to be more advantageous. The
Government will evaluate past performance based on the elements listed below:
6. INDIVIDUAL PROPOSAL RATING WORKSHEETS
6.1. A Proposal Rating Worksheet is provided as Attachment 4 that the evaluators will use to review and rate the
individual proposals.
6.2. During the consensus evaluation, a single ―consensus rating‖ worksheet shall be completed for each proposal
and signed by all the evaluators. Comments and supporting rationale for the rating assigned will
be included on the consensus sheet.
7. EVALUATION CRITERIA:
7.1. The Government will evaluate the information submitted in response to the evaluation factors listed in Para. 5.1
Corporate Relevant Experience, and 5.2. Offeror Past Performance. Both factors are considered to be equally
important.
8. EVALUATION STANDARDS.
8.1 Evaluation criteria (factors) will be rated using the following adjectival descriptions. Evaluators will apply the
appropriate adjective to each criterion rated. The evaluator’s narrative explanation must clearly establish that the
Offeror’s submittal meets the definitions established below. As each criterion is evaluated an assessment of
Performance Risk will be made. Performance Risk relates to the assessment of an Offeror's present and past work
and accomplishments to determine the Offeror's ability to successfully perform as required.
8.2 OUTSTANDING - Information submitted demonstrates Offeror's potential to significantly exceed performance
or capability standards. The Offeror's qualifications meet the fullest expectations of the Government. The Offeror
has clearly demonstrated an understanding of all aspects of the requirements to the extent that timely and highest
quality performance is anticipated. The Offeror has convincingly demonstrated that the RFP requirements have been
analyzed, evaluated, and synthesized into approaches, plans, and techniques that, when implemented, should result in
highly effective and efficient performance under the contract which represents very low risk to the Government. An
assigned rating of ―Outstanding‖ indicates that, in terms of the specific factor, the submittal contains no significant
weaknesses, deficiencies or disadvantages and presents very low risk that it will not be successful. The Offeror

possesses exceptional strengths that will significantly benefit the Government. Offeror very significantly exceeds
most or all solicitation requirements. Very high probability of success. Very low risk to the Government.
8.3 ABOVE AVERAGE - Information submitted demonstrates Offeror's potential to exceed performance or
capability standards. The Offeror's qualifications are responsive with minor weaknesses, but no major weaknesses
noted, and some strengths. The Offeror has clearly demonstrated an understanding of all aspects of the requirements
to the extent that timely and high quality performance is anticipated. Offeror demonstrates that the requirements of
the RFP are well understood and the approach will likely result in a quality of performance which represents low risk
to the Government. An assigned rating of ―Above Average‖ indicates that, in terms of the specific factor, any
weaknesses noted are minor and should not seriously affect the Offeror’s performance and presents low risk that it
will not be successful. The Offeror possesses one or more strengths that will benefit the Government. Offeror fully
meets all RFP requirements and significantly exceeds many of the RFP requirements. Response exceeds a
"Satisfactory" rating. High probability of success. Low risk to the Government.
8.4 SATISFACTORY - Information submitted demonstrates Offeror's potential to meet performance or capability
standards. The Offeror's qualifications are responsive with weaknesses in several areas that are offset by strengths in
other areas. The Offeror understands most aspects of the requirements and presents an acceptable solution to the
extent adequate performance is anticipated. Offeror demonstrates an understanding of the RFP requirements,
presents a complete and comprehensive proposal that will likely result in adequate performance which represents
moderate risk to the Government. A rating of ―Satisfactory‖ indicates that, in terms of the specific factor, the
Offeror may satisfactorily complete the proposed tasks, but there is at least a moderate risk that it will not be
successful. The Offeror possesses few or no advantages or strengths. Offeror meets all RFP requirements, presents
a complete and comprehensive proposal. No significant advantages or disadvantages. Offeror’s response exceeds a
"Marginal" rating. Moderate risk to the Government. In the case of no past performance on the part of the
Offeror, the Offeror must so state in the proposal. A “Neutral” rating will be assigned for Past Performance.
8.5 MARGINAL - Information submitted demonstrates Offeror's potential to marginally meet performance or
capability standards necessary for minimal contract performance. The Offeror's qualifications are generally
responsive but weaknesses exist that are not offset by strengths. The Offeror does not understand most aspects of the
requirements; the submittal is not adequately responsive or does not address a specific factor to the extent adequate
performance is not anticipated. Offeror does not understand the requirements of the RFP and although the submittal
has a reasonable chance of becoming at least acceptable, it requires modification to likely result in adequate
performance which represents high risk to the Government. The assignment of a rating of ―Marginal‖ indicates that,
in terms of a specific factor, mandatory corrective action would be required to prevent significant deficiencies from
affecting the overall project with high risk it will not be successful. The Offeror possesses no advantages or
strengths and some disadvantages. Offeror does not meet all the requirements of the RFP. Offeror displays low
probability of success, although the submittal has a reasonable chance of becoming at least acceptable. Offeror’s
response exceeds an "Unsatisfactory" rating. Significant disadvantages. High risk to the Government.
8.6 UNSATISFACTORY - Information submitted fails to meet performance or capability standards necessary for
acceptable contractor performance. The Offeror's qualifications have many deficiencies and/or gross omissions.
The Offeror’s understanding of the Government’s requirements is so superficial, incomplete, vague, incompatible,
incomprehensible, or incorrect as to make adequate performance very unlikely, or inconceivable. The Offeror does
not demonstrate an understanding of the RFP requirements and the submittal fails to provide a reasonable, logical
approach to fulfilling much of the Government's requirements. The Offeror fails to meet the minimum requirements
of the RFP and the requirements could only be met with major changes to the submittal. There is no expectation of
minimal performance which represents unacceptably high risk to the Government. The assignment of a rating of
―Unsatisfactory‖ indicates that, in terms of a specific factor, the Offeror fails to meet the minimum requirements and
there is unacceptably high risk that the overall project will not be successful. The Offeror possesses no advantages
or strengths and many disadvantages. The Offeror's does not meet most of the requirements of the RFP and the
proposal is so unacceptable that it would have to be completely revised in order to attempt to make it acceptable.

9.1 The Government evaluation team will consider all information provided in the proposal individually. Once
these individual analyses are completed, the team will meet and determine a rating by consensus decision.
9.2 Yes-No Ratings. Where the specific evaluation sheets indicate a YES-NO Rating, these items shall be treated as
information items. They are included in the evaluation worksheets to assure a similar focus among the evaluators
and to ensure that individual evaluators do not overlook proposal information provided.
9.3 ACCEPTABLE or UNACCEPTABLE Ratings.
9.4. ACCEPTABLE. The Technical proposal meets or exceeds minimum level of acceptability in the solicitation .
9.5. UNACCEPTABLE. The proposal does not meet the minimum level of acceptability.
9.6. As a result of the consensus rating, each proposal will be assigned a single overall Acceptable or Unacceptable
rating. This final overall rating will be provided to the Contracting Officer/Source Selection Authority.
9.7. It is the responsibility of the Source Selection Evaluation Board to document reasons for the Acceptable or
Unacceptable rating for the overall rating.
10. EXCEPTIONS. Exceptions to the contractual terms and conditions of the solicitation (e.g., standard company
terms and conditions) may result in a determination to reject a proposal.
11. RESTRICTIONS. Failure to submit all the data in the format indicated in this section may be cause for
determining a proposal incomplete and, therefore, not considered for evaluation, and for subsequent award.
12. PRICE.
12.1 The Government will perform a price analysis on all proposals received. Price analysis will be performed in
accordance with FAR 15.404-1, to determine completeness, reasonableness, and understanding of the work. The
evaluation will determine the adequacy of the offer in fulfilling the requirements of the proposal. Completeness
addresses the extent to which the elements of the price proposal are consistent with the requirements of the RFP.
Reasonableness will be established using historical price information, price competition information, the IGE, and
any other pricing tools necessary.
12.2 Price will not be scored, but will be a factor in establishing the competitive range prior to discussions (if held)
and in making the final best value determination for award.
13. BASIS FOR AWARD

13.2. Award will be made to the best overall proposal, which is determined to be the ―best value‖ to the
Government, with appropriate consideration given to the two factors: Relevant Corporate Experience and Past
Performance. To receive consideration for award, a rating of no less than ―Satisfactory‖ must be achieved for all
factors. Offerors are cautioned that the award may not necessarily be made to the lowest priced offeror.
13.3. Best value will be determined by a comparative assessment of proposals against all source selection criteria
in this RFP. In order to determine which proposal represents the best overall value, the Government will conduct a
tradeoff analysis, comparing the strengths and weaknesses of the competing proposals to determine which
proposal(s) represent(s) the best value to the Government and thus shall received contract award.

13.4. As technical ratings and relative advantages and disadvantages become less distinct, differences in price
between proposals are of increased importance in determining the most beneficial proposal. Conversely, as
differences in price become less distinct, differences in scoring and relative advantages and disadvantages between
proposals are of increased importance to the determination.
13.5. The Government reserves the right to accept other than the lowest priced offer. The right is also reserved to
reject any and all offers. The basis of award will be a conforming offer, the price or cost of which may or may not be
the lowest. If other than the lowest offer, it must be sufficiently more advantageous than the lowest offer to justify
the payment of additional amounts.
13.6. Offerors are reminded to include their best technical and price terms in their initial offer and not to
automatically assume that they will have an opportunity to participate in discussions or be asked to submit a revised
offer. The Government may make award of a conforming proposal without discussions, if deemed to be within
the best interests of the Government.

(a) The offeror's attention is called to the Equal Opportunity clause and the Affirmative Action Compliance

52.211-18 VARIATION IN ESTIMATED QUANTITY (APR 1984)
If the quantity of a unit-priced item in this contract is an estimated quantity and the actual quantity of the unit-priced
item varies more than 15 percent above or below the estimated quantity, an equitable adjustment in the contract price
shall be made upon demand of either party. The equitable adjustment shall be based upon any increase or decrease in
costs due solely to the variation above 115 percent or below 85 percent of the estimated quantity. If the quantity
variation is such as to cause an increase in the time necessary for completion, the Contractor may request, in writing,
an extension of time, to be received by the Contracting Officer within 10 days from the beginning of the delay, or
within such further period as may be granted by the Contracting Officer before the date of final settlement of the
contract. Upon the receipt of a written request for an extension, the Contracting Officer shall ascertain the facts and
make an adjustment for extending the completion date as, in the judgement of the Contracting Officer, is justified.

52.211-10 COMMENCEMENT, PROSECUTION, AND COMPLETION OF WORK (APR 1984)
The Contractor shall be required to (a) commence work under this contract within 30 calendar days after the date the
Contractor receives the notice to proceed, (b) prosecute the work diligently, and (c) complete the entire work ready
for use not later than 365 calendar days after receipt of NTP. The time stated for completion shall include final

(a) The Contractor shall procure and maintain during the entire period of his performance under this contract the
following minimum insurance:
Comprehensive and Employer's Liability Insurance in the amount
required by the State law in which the work is to be performed
under this contract.
Comprehensive General Liability Insurance in an amount not less
than $500,000 per accident.
Automobile Liability Insurance: $200,000 per person and $500,000
per accident for bodily injury liability and $20,000 property
damage liability.
(b) Prior to the commencement of work hereunder, the Contractor shall furnish to the Contracting Officer a
certificate or written statement of the above-required insurance. The policies evidencing required insurance shall
contain an endorsement to the effect that cancellation, or any material change in the policies adversely affecting the
interests of the Government in such insurance, shall not be effective for such period as may be prescribed by the laws
of the State in which this contract is to be performed and in no event less than 30 days after written notice thereof to
the Contracting Officer.
(c) The Contractor agrees to insert the substance of this clause, including this subparagraph (c), in all subcontracts
hereunder.

52.236-4020 PAYMENT FOR MOBILIZATION AND DEMOBILIZATION
(a) The Government will pay all costs for the mobilization and demobilization of all of the Contractor's plant and
equipment at the contract lump sum price for this item.
(1) Sixty percent (60%) of the lump sum price upon completion of the contractor's mobilization at the work site.
(2) The remaining Forty percent (40%) upon completion of demobilization.
(b) The Contracting Officer may require the Contractor to furnish cost data to justify this portion of the bid if the
Contracting Officer believes that the percentages in paragraphs (a) (1) and (2) of this clause do not bear a reasonable
relation to the cost of the work in this contract.
(1) Failure to justify such price to the satisfaction of the Contracting Officer will result in payment, as determined by
the Contracting Officer, of --
(i) Actual mobilization costs at completion of mobilization;
(ii) Actual demobilization costs at completion of demobilization; and
(iii) The remainder of this item in the final payment under this contract.
(2) The Contracting Officer's determination of the actual costs in paragraph (b)(1) of this clause is not subject to
appeal.
