This form is the first page of the thesis, and is typed by the Graduate College using the information
submitted on the Committee Recommendation Form. After a successful defense, the student’s
committee and department sign the form and give both copies to the student for use in the thesis. A
page number does NOT appear on this page, although it is page i in sequence.
