NOMINATION GUIDELINES
There are 18 award categories, 17 of which may be nominated for using this form. Each award has its specific criteria, however all awards must recognise
outstanding achievements or contribution towards the sport industry in NSW in 2011, at an international and/or national and/or state level.
The nomination form must be submitted by a NSW sporting organisation who is a member of NSW Sports Federation.
Organisations may submit only one nomination for each category. The exception is for sporting organisations which represents several different sports.
These nominations must be ranked by the organisation.
All fields must be completed on the nomination form and additional documents submitted will not be considered.
The nomination form must be accompanied by a high-resolution jpeg image (350KB or higher).
Nomination forms and accompanied files will not be returned after judging.
The NSW Sports Federation may use information provided in/with the nomination for publicity. By submitting a nomination the organisation gives
permission for this information to be released.
The Federation reserves the right to add nominations and may cancel an award category if insufficient nominations are received.

Nominations close at close of business (5pm) on Thursday 1 December 2011.
AWARD CATEGORIES
NSW Athlete of the Year become nominees for the NSW Volunteer of the Year award.
Awarded to an individual athlete who has achieved outstanding Volunteer nominations outside of the NSW Sports Volunteer
results in 2011. Awards will not be accepted.
NSW Team Athlete of the Year NSW Sports Media Award
Awarded to a team athlete who has achieved outstanding results Awarded to an individual or organisation that has displayed
in 2011. The team athlete must compete only as part of a defined outstanding contributions to the wider coverage of sport in NSW in

NSW Young Athlete of the Year
