Insert Sheet for Summary Plan Description Concordia Retirement Plan Changes Effective January 1, 2006, February 27, 2008, and June 1, 2008
Concordia Retirement Plan — The following changes are made on the listed pages of the Summary Plan Description (changes shown in italics): Page 13 – Effective June 1, 2008, retired members can name a beneficiary or beneficiaries to receive the $10,000 lump-sum death benefit in lieu of their spouse or child(ren). Under the heading “Survivor Lump-Sum Benefit—Death of a Member,” replace both paragraphs with the following: Upon your death after retirement, a lump-sum death benefit of $10,000 is paid to your designated beneficiary(ies). If there is no beneficiary designation on file with Concordia Plan Services at the date of your death, or if all of the designated beneficiary(ies) predeceased you, the benefit will be paid to your surviving spouse, if living; otherwise to your surviving children, in equal shares, if living; otherwise to your estate. Page 18 – Effective January 1, 2006, located on the side bar on page 18, replace the paragraph with the following: If the Employer joined The Church’s Plan effective anytime between 1/1/2006 and 1/1/2007, you may qualify for RMS Initial Service Credits based on your past years of CRP and PPPT participation. Page 18 – Effective February 27, 2008, under the heading “Who Is Eligible,” change the second bullet to read: n You must be enrolled in the Concordia Health Plan unless you qualify as an Eligible Opt-out because you do not meet your Employer’s minimum hours worked requirement or are already covered by: n A spouse’s group health plan (coverage by virtue of employment, including military service) n A parent’s group health plan (coverage by virtue of employment, including military service) n A former employer’s group health plan n A current non-LCMS employer’s group health plan n The military as a retiree n A Medicare supplemental plan n A government medical assistance program for low-income persons (e.g., Medicaid) n A foreign country plan n A group health plan of an LCMS employer located in the state of Hawaii Page 29 – Effective February 27, 2008 the above changes made to the definition of Eligible Opt-out on page 18 should also be made to the definition of Eligible Opt-out under the “Common Terms” heading.
