GAP ANALYIS TEMPLATE
GAP Analysis is a tool that can be used to compare projected performance against
actual performance.
GAP Analysis is used to identify areas and/or processes that require improvement.
Identifying what an organization’s goals are and how to achieve them consists of
identifying gaps in projected solutions and in processes.

Gap Analysis Template:
(Use this template as a tool to assist you in evaluating where your organization now stands and
where your organization wants to be, per the determined time frame goal.
To use this template, (1) date your template. (2) Follow through on each item with a “Yes” or
“No” checkmark and any comments; if there is an item that is not applicable, note “N/A” for the
item. (3) Go through the “No” checkmarks with appropriate company representative(s,) and
obtain the necessary information. All items should be checked as “Yes” or “N/A.”
Wherever indicated, include quantitative calculations.)
