The Diabetes Educator Section of the Canadian Diabetes Association, in conjunction with Bayer
Inc., is pleased to announce the Bayer Awards, which may be awarded annually. Formal
applications for the Bayer Awards are invited from members by April 1st each year.

Selection of candidates is the responsibility of the Bayer Award Selection Committee of the DES.
The Vice President, Research & Professional Education and/or Director, Professional Development,
and the Manager, Bayer Inc. will sit as non-voting committee members.

TERMS OF REFERENCE
1. Applicants must be DES members in good standing. The principal investigator must be a DES
    member in good standing for a minimum of 6 months.
2. Two awards will be available each year. The Bayer Award for Applied Education Research must
    be used for innovative educational research. The Bayer Award for Education Innovation must be
    used to development educational material that will benefit persons with diabetes.
3. Each award (up to $5,000) may be given annually. One-third of the monies will be issued initially,
    one-third after the interim report, and one-third upon completion of the project and submission of
    the final report, including financial statements, to Bayer and the DES. The award is not intended
    to be used for personal support, salaries for clerical support, or travel/conference expenses,
    however, up to 20% of the amount awarded may be considered for professional/clerical support.
4. Within 1 year of receiving initial funding, a progress report must be submitted to the Coordinator,
    Professional Membership & Projects, and every year thereafter until the funded project is
    complete. This project must be finished in 2 years in order to receive the remaining funds.
    Extensions may be granted on an individual basis, as reviewed by the DES Executive Awards
    Committee. Final payment is awarded when an article on a project has been accepted for
    publication by either Canadian Journal of Diabetes or Diabetes Quarterly.
5. The project design should be the original concept of the applicant.
6. In any given year, a project may be submitted for only 1 award.
7. A paper must be submitted to Canadian Journal of Diabetes by award recipients within 6 months
    of completion of the projects. If rejected for publication in the journal, an article must be
    submitted to Diabetes Quarterly for consideration.
8. Any scientific paper or poster resulting from the work of award recipients shall be acknowledged
    through the policies of the Canadian Diabetes Association (CDA) and Bayer Canada Inc.
9. Bayer Inc. retains the right of first refusal over sponsorship of material developed within the
    award project, which is to be made available for distribution to diabetes educators.
10. Principal award recipients will be recognized at the CDA/CSEM Professional Conference and
    Annual Meetings. Bayer Inc. will contribute expenses for recipients to attend the meeting if no
    alternate funding is available.
11. All applicants will be notified of the selection results. Unsuccessful applications will be destroyed
    1 month the awards are selected. The names of unsuccessful applicants will be kept on file for 1
    year. Unsuccessful applicants may be asked to resubmit their application.
12. Any awards project that is published must include the following statement: “The opinions
    expressed in the project are those of the author(s) and have not been endorsed or approved in
    any way by the Diabetes Educator Section of the Canadian Diabetes Association or by Bayer
    Inc.”

                                 APPLICATION INSTRUCTIONS

1. Applications must be typewritten and received in their entirety at the Canadian Diabetes
   Association national office by April 1. No late submissions will be accepted. Please send 10
   copies of your application (1 original, 9 blinded) to:

2. Attach a completed Cover Sheet to your application. The Cover Sheet is retained at the
   CDA national office for the purpose of circulation of applications to the Bayer Award
   Selection Committee.

3. Do not identify yourself or your affiliation in any way in the application form, or you may
   jeopardize your success in obtaining funding. The Selection Committee conducts a blinded
   review of each submission, rating applications according to established criteria. This process
   ensures applications are judged solely on their own merit and eliminates any bias by the
   Committee.

4. Any Bayer Award Selection Committee member(s) involved with a particular application or
   support of a specific application will be asked to withdraw from committee proceedings
   during discussion of that application.

5.   Complete your application by following the Application form outline:
a)   Title of the project.
b)   State the purpose, specific objectives and underlying hypothesis of the project.
c)   Describe the relevance of the project to people with diabetes as well as the improved
     outcomes expected.
d)   Describe in detail the project design and outline specific plans for implementation and
     evaluation.
e)   Include relevant references on the project hypothesis and area of research. References and
     abbreviations should follow the Uniform Requirements for Manuscripts Submitted to
     Biomedical Journals (CMAJ. 1997;156:270-285) and Index Medicus.
f)   Give a projected schedule and completion date for each phase of the project.
g)   Outline the projected budget. State total costs for the project and itemize specific
     expenditures. Applications budgeted for >$5,000 must show plans for seeking additional
     funding. Acceptance of a Bayer award will be conditional on evidence of application for
     additional funding. Within 1 month of notification of winning award you must demonstrate
     evidence of additional funding being secured.
h)   Where applicable, letters from approval bodies (such as your institution's Ethics Committee),
     should be submitted.
