708 2010 1S HiPo unstable and nearly capsized.
Workboat engine stalled just after launch. WB drifted into superwide rope and capsized.
All three crew were recovered safely. Chafing had made a hole in a fuel line. Before
launch a considerable amount of fuel and water had been drained from the bilge, but
709 2010 1S HiPo crew failed to notice this warning sign!

Engineer removed water pump cover, without checking that the suction valve below it had
been properly closed. The suction valve had actually failed and large amounts of water
started flowing into engine room. Bilge pumps initially failed to start. Main sea water
intake valve had to be closed, cover was quickly put back on the pump, after which sea
water cooling to other engines could be restored, before these shut down. Potential for
710 2007 5M HiPo catastrophic water intake into engine room and subsequent sinking of the vessel.
Wooden fishing boat, unlit, ignored chase vessel instructions and crossed spread, nearly
711 2007 5M HiPo hitting the superwide rope, which would have caused it to capsize.
High pressure air manifold for airguins exploded on being re-pressurized. Gun lubrication
had not been stopped during maintenance, leading to oil build up and "diesel" effect.

1. Employee was not wearing his PPE (life vest) in an Geophysical Marine Streamer handling Boat MOB
area and a job procedure that required it. Employee
was also known to all as a non-swimmer.
2. The job procedures at that time had the cable
hands coiling up the buoy rope on the deck and
feeding it out from the aft deck after the last cable
head was deployed.
After the investigation of this fatality the use of buoy
rope canisters was initiated so no buoy rope was
ever to be coiled up and fed off of the deck during a
cable deployment again.
