WHEN AM I ELIGIBLE TO PARTICIPATE IN THE PLAN?
You are eligible to participate in the Plan on the 1st of the month following 30 days of full-time
employment or have worked 1000 hours of service during a 12-month period and you are 21 years old.
You may enter the Plan on the 1st of any month following completion of the service requirements.
WHAT CAN I CONTRIBUTE TO THE PLAN?
You may contribute up to 24% of each paycheck before-tax dollars to your 401(k) account up to the
maximum allowed by law. You are always 100% vested in your contribution. For 2005, the IRS maximum
is $14,000. Participants age 50 or older may contribute an additional $4,000 “catch up” contribution
during 2005.
WHEN CAN I CHANGE OR STOP MY CONTRIBUTIONS?
You may change (increase or decrease) your contributions through Fidelity’s Online Service
(www.401k.com or 1-800-294-4015) at any time. The change will be effective on the next pay period.
You may stop your contributions at any time, but must wait until the 1st of the month to re-enter the plan.
WHEN CAN I CHANGE OR STOP MY INVESTMENT ELECTIONS?
You may change your investment election at any time utilizing Fidelity’s Online Service. (www.401k.com
or 1-800-294-4015)
WHAT WILL THE COMPANY CONTRIBUTE TO THE PLAN?
Moroch/Inspire currently makes a 25% matching contribution to your account, beginning the 1st of the
month after one-year from hire date.
WHEN DO I VEST IN THE COMPANY MATCHING CONTRIBUTIONS?
You vest in the value of your company matching contribution account based on your years of service.

WHEN WILL I RECEIVE MY VESTED ACCOUNT BALANCE?
You can receive your vested account balance only upon death, disability, retirement, hardship, or
termination of employment.

SINCE MY CONTRIBUTIONS TO THE PLAN ARE TAX-DEFERRED, WHEN DO I
PAY TAXES ON THE DOLLARS?
When you take a distribution from the Plan as a result of termination of employment, disability, retirement
or death, the distribution will be taxable income to you. IRS intends for this to be a retirement Plan, and if
you take a distribution prior to age 59 1/2, you may also have to pay an additional 10% excise tax. You
will be able to roll the distribution into an IRA or may be able to roll it to another Employer Qualified
Retirement Plan, or take a lump sum distribution from the Plan.
Distributions which are not rolled into an IRA or another employer’s plan are subject to mandatory 20%
IRS withholding tax which represents a deposit toward the tax due on your distribution.

HOW DO I ACCESS FIDELITY’S ONLINE SERVICES?
You can access online services at www.401k.com for a variety of services. Once you establish your
password you can change your contribution amount, review or change you investment selections, review
or print a statement of your account or apply for a loan. You also have access to retirement and
investment sources.

INVESTMENT ELECTION OPTIONS
Moroch/Inspire’s 401k Plan is serviced by Fidelity Investments and is automated through their on-line
website @ www.401kexpress.com. An enrollment kit will be mailed directly to your home by Fidelity upon
receipt of your new hire information to Human Resources at Moroch/Inspire. Once you receive your
packet from Fidelity you will be able to enroll in Moroch/Inspire’s 401k plan. Please contact
humanresources@moroch.com if you have additional questions. A list of our current investment options is
