The evaluation form consists of 18 core course evaluation questions and 17 instructor
evaluation questions (List of questions available on the faculty Blackboard). It may be possible
for you to add up to 5 custom questions to your course evaluation. Please list desired custom
questions in the space below, or schedule an appointment with Kim Deloatch
[kim_deloatch@unc.edu) to discuss these. You may also use the survey feature in your course
Blackboard to ask students additional specific questions about your course.
Instructor Evaluations
A maximum of five instructors, including the course director, may be evaluated. Please consider
limiting this list to those instructors who have taught more than five hours in the course or need
an evaluation for promotion and tenure purposes. It is your responsibility to discuss the
evaluation process with these individuals. Teaching assistants should NOT be included in this
list. We can set up a separate evaluation survey for TAs at your request, or you can create a
TA evaluation survey in your course Blackboard.

Incentives for Completion of Course and Instructor Evaluations
Course Directors have complete authority over whether they will offer any incentives for survey
completion (or penalties for non-completion). Historically, survey response rates are
significantly (20-25%) higher in courses with incentives. The published deadline for completing
Spring 2011 evaluations will be May 23, 2011. By nature, most incentives are given for early
completion and involve grades (thus must be submitted prior to the close of the semester). The
list below is a sample of incentives that instructors have offered in the past. You may choose
one of these, write your own, or provide no incentive. If you do offer an incentive for early
completion, a staff member will send you a list of students who completed the evaluation on the
day after your chosen deadline. For this reason, please do not choose a Friday or Saturday
deadline.
