ORIENTATION FOR FACULTY & STUDENTS
INTRODUCTION:
Welcome to The Queen's Medical Center. We hope that your clinical experience at QMC will be
rewarding and memorable. If this is your first time at QMC, contact the Clinical Affiliations Coordinator
at (808) 691-4235 for an orientation to the medical center. Undergraduate students receive orientation on
their first clinical day by their faculty. Faculty new to Queen’s should make an appointment with the
coordinator for orientation to our facility.
Clinical Affiliations forms are included in this document. These forms are required for all of our
clinical affiliation groups and should be submitted within one week of the start of your practicum. Failure
to do so may result in an interruption of the practicum until all documents are received.
CONFIDENTIALITY:
Students and faculty must provide written verification that they have completed QMC approved
HIPPA education, which includes information about patients’ right to privacy and appropriate use of
protected health information for educational purposes.
All patient information accessed by an affiliating student or faculty member in the performance of
their duties is considered "CONFIDENTIAL". It is the moral and ethical responsibility of all staff to
maintain the confidentiality of all patient information.
Protection of patient confidentiality extends to the use of all patient data / forms, as well as to the
disclosure of identifiable patient information, whether by oral, written or electronic means.
Patients understand that QMC is a teaching facility, and students should introduce themselves to
their patients prior to initiating care or reviewing patient charts. Patients maintain the right to refuse
having students participate in their care.
Documents with patient identifiers should not be removed from the QMC campus,
including reports generated from the electronic medical systems. Any duplication of these
records electronically or by photocopy is forbidden.
Copies of the Patient Care Summary may be printed for use by students to deliver patient care during
their assigned shift, but must be properly disposed of in the LOCKED RECYCLING BINS at the
completion of their shifts.
CLINICAL AFFILIATIONS INFORMATION FORM:

1. Must include Course information, clinical dates and times, contact information, BLS certification,
Immunization records, TB testing & CXR if indicated, RN license number and Liability
Insurance (if not provided through school) See QMC Forms.
2. Submit to Patient Care Consulting Services at least one week prior to the first clinical day.
(FAX: 691-7763) Schools may submit student / faculty health and CPR records using their
institution’s attestation form in lieu of this document.
NOTE: A delay in the start date for your group or individual students may occur if the
information requested is not received or current (CPR, TB testing must be current for the entire
time students are at QMC). QMC requires students to renew their BLS every 2 years.
TUBERCULOSIS MONITORING FORM: (QMC Forms Each instructor and student with a positive
PPD and negative chest X-rays must complete one of these forms annually and submit it as a supplement
to the Clinical Affiliations form.
MEDICAL RECORDS INFORMATION SHEET: (QMC Forms)
1. The form must be signed by the INSTRUCTOR and each STUDENT who will be at QMC for
clinicals.
2. Submit original form to the Clinical Affiliations Coordinator by the first clinical day. Students
will be denied access to patient records in the Medical Records Department if this form is not
completed or access is requested after the end date of the practicum.
CLINICAL AFFILIATIONS SAFETY AND HIPAA TRAINING DOCUMENTATION FORM:
(QMC Forms)
1. Faculty or coordinator will review answers to the 2011 version of the Supplement on Safety
(SOS) self-learning module with students and enter the date of completion on the Safety &
HIPAA Form (QMC Forms)
Please do not return individual student tests unless instructed to do so. Use this form to also
verify that QMC approved HIPAA training was completed for each student prior to start of the
practicum. Breaches of patient confidentiality by students may result in the immediate termination
of their practicum at QMC.
2. Note: The signed Student Confidentiality and Access Agreement (QMC Forms) validates the
completion of required HIPAA training and should be kept on file by the schools for a period of
six years.
4. Faculty: Please return the SOS and HIPAA Training Form to the clinical affiliations
coordinator after reviewing the correct answers with your students.
CORPORATE COMPLIANCE TRAINING :

Corporate Compliance training is mandatory for any student or faculty who will be doing >160
hours of practicum at QMC per calendar year. Access codes will be provided for training, which
can be accessed on the Internet when hours approach 160 hours.
It is the responsibility of the student and faculty to notify QMC coordinator when an individual’s
total clinical hours at QMC will exceed 160 hours.
UNIT ORIENTATION:
Clinical Faculty are responsible for contacting the Clinical Affiliations Coordinator for a tour and
a faculty orientation session. In addition, Clinical faculty must contact the manager of the unit(s) where
their clinical groups will be scheduled to arrange for unit specific orientation. This includes, but is not
limited to, patient care delivery systems, documentation, physical layout and communication. If you need
assistance contacting the manager of any unit(s), a directory has been provided for you in this packet or
contact the Clinical Affiliations Coordinator at 691-4325.
CARE*LINK ACCESS AND DOCUMENTATION:
The Queen’s Medical Center utilizes an electronic patient care management (EMR) system called
“Care*Link”. The patient records will be electronic with the exception of consultations and specialty
forms.
For this reason, failure to attend Care*Link training and to have a Care*Link student sign-on
code may result in denied access to clinical practicum at QMC. The exceptions to this will be for students
enrolled in a Nursing Fundamentals I or other like experiences as deemed appropriate by both the course
coordinator and unit managers.
Students should review their progress notes with their preceptor or faculty before entering it into
the EMR. Student charting will automatically generate a “cosign required” link for the instructor.
However, the preceptor should also have an opportunity to review the note before entry into the EMR.
In the event that Care*Link is down, students will continue to document their patient care
information on the Downtime form available on each unit.
CARE*LINK FOR INSTRUCTORS & STUDENTS WITH PREVIOUS QMC CARELINK
TRAINING:
1. Students or faculty who are employees of QMC: Depending on employee job description,
Care*Link training may not be required (check with clinical coordinator). Students and faculty
should not use their employee sign-on codes during their clinical time at QMC. A separate
Sign on code for the employee to use as a student or faculty. A new Computer Access
Request (CAR) Form (QMC Forms) must be submitted to reactivate sign-ons each semester.
2. Students or faculty who were previously trained at QMC and received a student or faculty
Care*Link sign-on code: No retraining necessary. However, Clinical Faculty should instruct
students with existing Care*Link student sign on codes to complete and submit a new CAR Form
to the CareLink Training Center at least one week prior to start of clinical in order to reactivate
their sign on codes.
3. IMPORTANT: Instructors and students must update their sign on each semester by
submitting the signed, hard copy Computer Access Request (CAR) Form (QMC Forms) to:

MEETING ROOMS: Space is limited, and we recommend the following to reserve meeting rooms.
First, contact the manager of the clinical area and inquire about scheduling the use of conference space on
the nursing unit. If no conference space is available on the unit, you should submit a request to the QMC
Help Desk as soon as possible. Simply e-mail your request to helpdesk@queens.org. Please be sure to
include date, start time, end time and number of chairs required. You may also contact the HELP
desk by calling 691-HELP.
NOTE: Please cancel all room reservations you will not be using by calling the Help Desk at
691-4357.
REQUESTS FOR AUDIOVISUAL EQUIPMENT:
1. Most units have a TV and a VCR/DVD. See unit managers for availability.
2. Patient Care Consulting Services has limited AV equipment for use during normal working hours,
however, priority is given to QMC scheduled classes or inservices. Please check with Clinical
Education at x4731 regarding availability of equipment and instructor support.
PARKING:
Queen’s does not provide subsidized student parking. Parking options are limited:
1. Car pooling or Bus.
2. Street parking available above the freeway.
3. Neil Blaisdell Center parking lot. Opens at 5:30am. All day parking for $6.00. Call the NBC
Parking Office at 592-2616 prior to the end of each week for days that parking will not be
available due to special events. Students are permitted to use the services of the QMC shuttle.

1. Shuttle times are from 5:30 a.m. to 9 a.m. and 2 p.m. to 7 p.m. daily, Monday to Friday, except
holidays and when the Blaisdell Parking is closed.
A recent change now provides a "Scheduled Shuttle Pick-up" beginning at 7 p.m. each day that the shuttle
provides service until shuttle service ends for that day (approx. 2 a.m.). The pick up times will be at quarter past
the hour and quarter to the hour. For example, pick-up at the ER Ambulance Dock for evening and night service
will be at 7:15 p.m. and at 7:45 p.m. The round trip to Blaisdell Parking Lot and Aala Street Parking Lot and back to
QMC takes 20 to 25 minutes depending on traffic. With this change, employees will no longer have to come down
to the pick-up site and perhaps wait 25 minutes for the shuttle to return.
2. The van / car make round trips between the QMC Porte Cochere, the Bus Stop on King Street fronting the
Blaisdell Concert Hall and Aala Park Parking Structure.
3. Riders must wait for the van at the above locations. A round trip takes approximately 15-20 minutes or
more depending on traffic. There are no set times for the van to be at either location during round trip runs.
4. When no one is at the van stop, the driver is instructed to wait at the stop for 5 minutes before continuing.
5. It is requested that all riders wait until the driver sets the step before getting into or out of the van to prevent
injuries.
6. People who have authorization to ride the Shuttle Van are all QMC employees and various groups of QMC
associates. In all cases, a rider must have an authorized QMC ID Badge or Nursing School ID Badge.
7. Riding the van is on a "First Come - First Served" basis.
Parking is free on Sundays and QMC Holidays in Miller and POB 1.
QMC will continue to validate parking for instructors. There will be no charge with validation.
Instructor parking is validated for the Miller Street Garage and POB 1 garages.
The Queen's Medical Center will not be responsible for costs incurred for theft or damages to
your vehicle. Unauthorized use or transfer of parking validation is prohibited and may result in
termination of the practicum.
