On behalf of the Mason City Country Club, we would like to thank you for your interest in joining our
Club. We offer several different membership categories and excellent facilities to meet your needs. These
facilities include an 18 hole golf course, large driving range, Pro Shop and restaurant as well as a beautiful
pool and cabana.
Our Chef and staff will present delicious menus and desserts for your dining pleasure. The dining
facilities offer both formal and casual fare that can be used for either personal or business functions and ac-
commodate parties of 15 to 420. Our full service Pro Shop offers a PGA Professional &
apprentice with services ranging from golf instruction to cart rentals and a full line retail shop for all your
golfing needs.
We would like to offer membership at the Mason City Country Club facilities to you and our family.
The Club offers many member functions and tournaments throughout the year for members of all ages. We
look forward to welcoming you to our warm and friendly environment. You will find membership at the Club
is a great way to meet people in your community.

Membership Agreement 2011
The following are the available categories of memberships:
A) 32 Years Old and Under - Full Golf
$103.00 per month - Class 4 Membership
*In the year of the 33rd birthday, the member can convert to the ongoing Progressive
Membership
B) Five-Year Full Golf Progressive
Year l: $99 per month - Class 6 Membership
Year 2: $136 per month - Class 7 Membership
Year 3: $177 per month - Class 8 Membership
Year 4: $200 per month - Class 9 Membership
Year 5: $212 per month - Class 10 Membership
*No Current Assessment. Minimum spending waived for first calendar year.
C) Widow/Widower/Single (Living in a One-Person Household and Unmarried) Full Golf
$139 per month - Class 5 Membership
* $5 Per Month Assessment-Course Improvement Fund
The membership classifications listed above are subject to purchasing a Membership Certificate in the
amount of $500. The Membership Certificate may be paid in full upon acceptance as a member, or the
member my elect to pay $250 per year for the first 2 years of membership.
D) Social/Pool Membership-$48.00 per month – Class 3 Membership
*No Membership Certificate, No Current Assessment
*All Dues are billed monthly, January through December. Members may opt to pay the 12 months of dues as a lump sum upon
their joining the Club or in January of each year. Iowa Law requires that sales tax for the entire year be billed in January or the
month of joining regardless of whether or not you elect to pay a lump sum or monthly dues.
**All Dues are subject to future changes.
MINIMUM SPENDING POLICY: No minimum spending is required for Membership Classification 4. Minimum spending shall be in
the amount of $100 each bi-monthly period for Membership Classification 5 and $130 all other membership classifications. The
minimum will be billed every two months from March through December. There shall be five periods consisting of March-April,
May-June, July-August, September-October and November-December. The minimum spending is not a monthly requirement and
the required amount may be spent at any time during the two month period. Minimum spending does not apply to the months of
January and February.
*Minimum Spending requirement is waived for the first year of membership-through December 2011

SPECIAL 2-YEAR PROGRESSIVE MEMBERSHIP COMMITMENT
If accepted as a progressive member, I agree to abide by the Constitution, By-Laws, and Rules of Mason City
Country Club, now in force or hereafter to be enacted.
I understand the current Membership Certificate fee (exempt for Social Membership) to become a golf
member is $500 to be paid either in full upon acceptance of membership application, or in three (3) yearly
installments beginning at the end of the two (2) year commitment. I understand the monthly dues and fees
for _________membership are currently $_______ per month.
As a part of a special membership promotion, Mason City Country Club has agreed to accept $_______ per
month for monthly dues and fees for the first year ending December 31, 2011. I understand and agree that
after the first 12 months I will pay regular monthly dues and fees that apply which is $136 for golf members
and $48 for social members starting January 1, 2012. If the progressive member wishes, they can combine
the first and second year dues and fees and make 24 equal payments.
In consideration of this special membership promotion, I further understand and agree to a minimum
membership of 2 years. If I resign from my membership for any reason other than a full-time job
transfer more than 50 miles from Mason City Country Club, I will be responsible for all dues and fees
for the 2 year period.
I understand that I may resign from the Club after 2 years at any time on condition that I have:
(a) Tendered my resignation in writing to the Secretary of the Club at least 15 days prior
to the first day of the month in which the resignation is to become effective;
(b) Paid all indebtedness owed to the Club; and
I acknowledge I have been given the opportunity to review the rules and policies of Mason City Country
Club, understand the current food and beverage minimums, and that I am responsible for any charges made
by my family members or my guests.
I understand false or misleading information given in my application may result in the application being
denied or membership terminated. I further agree to be responsible for attorney fees and collection costs in
the event my account becomes delinquent.
