1. Look Up & Print Your Tax Record – If your county is not listed
   below please revert back to Flate Rate Counties available tab on website.

   A.  PASCO COUNTY –http://appraiser.pascogov.com/
      1. Click on Records Search at the bottom of the page
      2. Go to bottom and review and accept agreement
      3. Go to address; fill in and click on submit
      4. You will be able to view your tax record at this point. Print and/or save to
         your computer a copy to help fill in the Listing Agreement.
   B.  PINELLAS COUNTY –http://pao.co.pinellas.fl.us/
      1. Go to the bottom of the page and click on Search Our Database
      2. Go to bottom of the page; fill in address and click on Look Up Address
      3. You will be able to view your tax record at this point. Print and/or save to
         your computer a copy to help fill in the Listing Agreement.
   C. HILLSBOROUGH COUNTY –http://www.hcpafl.org/
      1. Click on Property Record Search on the top middle banner
      2. Fill in owners name and click below search by name
      3. Click on your name and make sure it is your address
      4. You will be able to view your tax record at this point. Print and/or save to
         your computer a copy to help fill in the Listing Agreement.

2. Read Limited Agreement, Addendum #1

  A. If you have a USER Code fill the green box in upper right corner in at top right of
     page 6.
  B. Fill in COUNTY for which your property is located.
  C. Fill in all Seller’s Information that is in red.
  D. Fill in all yellow areas with legal names on tax record with actual signature(s)
     after printing the agreement.

3. Listing Plans Page
  A. Pick a Flat Rate Listing Plan that fits your Sale, Rental or Lease Option. $99.00
     for Plan I. $299.00: Plans II & III
  B. All property will be sold as is with right to inspect.
  D. Must have our sign. Sign will say Quantum Realty Solutions For Sale with Owner
     and phone number. You may install your own flyer box.
  E. All yellow areas must include actual signatures after the agreement has been printed.

4. Sign One Time Set Up Fee form

7. Limited Service Listing Agreement

  A.   Make NOTE on line 7B, seller can sell the property without paying any
       commission to a private party that is not represented by any Real Estate Licensee.

  C.   Section 1 – Fill in all lines. Add 5 days to termination date (For set Up time on
       $99 Plan I)
  D.    Section 2 – Fill in all lines. Make note of C. List all items staying (add an

11. Lead Paint

  A.   If your property was built before 1978, fill in all areas with asterisks with names
       on tax records, sign and date in Seller’s area highlighted in yellow with legal
       signatures after printing the agreement.
                        Disregard if your home was built after 1978.

12. Sellers Real Property Disclosure Statement
       & Mold Addendum
  A.   Answer all questions the best you can.
  B.   If you never lived at the property, fill in the best you can and write a statement
       explaining that you have never taken residency at this property.
  C.   If an association takes care of roof, etc, answer Association.
  D.   Number 7: find out if it is in a Flood Zone. (You can find this out from the county
       tax records or your Insurance Company.)
  E.   You will print the document and apply all legal signatures and dates to the
       highlighted yellow area.
  F.   All yellow areas must include actual signatures after the agreement has been
       printed.

13. Residential Profile Sheet

  A.   Fill in ALL areas.
  B.   Owner Information (If there is a current tenant please make note if it is o.k. to
       contact this party) This information will be noted in Realtor Remarks (not public).
  C.   Fill in sign information.
  D.   Sign and date all yellow areas after document has been printed.
  E.   Read Note under Residential Features and then check appropriate boxes.
  F.   If there is a tenant we will add name and contact information, if o.k. in Realtor
       Remarks for other Realtors to see only.

  G. Driving Instructions
     1. Start Directions from a main road and indicate whether to go North, South,
        East or West, left or right and if needed any landmarks.
  H. You will print the document and apply all legal signatures and dates to the
     highlighted yellow area.

15. Lock Box

17. Credit Card Authorization Form

  A.   Fill in all information regarding your credit card.

22. Change in Plan

  A. We will allow you to have Quantum Realty Solutions represent you for a .5%
     (½%) of sale price at any time.
  B. Just fill in the Listing Agreement Commission form and fax to 813-854-2700.
     (Form found online also).

23. Sign

  A. NOTE! Our sign must be used with Plan II & III. You elected to offer a 2%(+)
     commission only to the Realtor that sells your property. Realtors will not show
     your home with a For Sale By Owner sign, they will be afraid that their Buyer
     will bypass them.
  B. You may sell your home yourself by:
         Ø Placing a spec sheet on the sign with your address and contact information
         Ø Hold open houses
         Ø Advertise in the paper
  C. If your community has a special sign you must have it made to say

2. SELLER’S RESPONSIBILITIES AND COMPLIANCE: Seller acknowledges that Listing Broker is subject
to and will abide by the various federal, state and local rules and regulations of the Multiple Listing Service,
and agrees to assist Listing Broker's compliance with all such rules and regulations. Seller understands Listing
Broker is subject to fines and sanctions for noncompliance of MLS rules and regulations. Should Seller fail to
provide the below information or default under any terms and conditions of this Agreement, Seller must pay
expenses and any fines imposed on Listing Broker by the MLS or any other agencies for noncompliance,
including reasonable attorneys' fees incurred in connection with such default or the enforcement of this
Agreement. Additionally, failure to comply may lead to the MLS listing being removed from the MLS at the
Listing Broker's discretion at any time during the listing time period (per listing agreement).

          You agree to provide ListingBroker the following in a timely fashion:
          1. Prior to your MLS listing going active:
                   . a photograph of the property for sale………
          2. Within 24 hours of your home going under contract:
                   . a copy of the executed sales contract, lease option or rental contract….
                   . the name of the Realtor representing the Buyer and their phone number…
                   . the name of the brokerage the Buyer's Realtor works for…..
          3. Within 1 week of your home going under contract:
                   . the name and phone number of the title company or attorney handling the closing..
          4. Within 48 hours of your closing:
                   . a copy of your HUD-1 Settlement Statement from the title company or attorney…
                    . your forwarding address and contact information…….

3. PHOTOGRAPHS: All photographs (maximum 6) are free on the MLS listing and one is required in
order to add the listing to the MLS. Listing Broker reserves the right to remove your listing from the MLS for
non-compliance. Any fines assessed to the Listing Broker are paid by the Seller, as referenced in section 2.
Planet Realtor has one (1) picture posted to MLS, on all local MLS listing, will have up to six (6) pictures.

4. USAGE: Usage of the Listing Broker's service of listing on the MLS is a privilege, not a right, and it is the
sole discretion of Listing Broker to determine who may utilize this service. Furthermore, Listing Broker
reserves the right to terminate Seller's MLS listing privileges at the Listing Broker's discretion at any time
during the listing time period. Once a property is listed on the MLS, no refunds are issued. Once property is
under contract, the listing will stay on MLS until property closes or contract falls through. $99.00 listings
under contract will be charged each month until property closes (no prorations). $299.00 listings under

contract will not be charged any additional month’s fees until property has closed or listing is cancelled.


5. TITLE: Seller warrants having legal authority and capacity to convey the Property with all
improvements. If purchaser for the Property is secured, the usual and customary practice is for the title
examination, curing the title and for closing the transaction shall apply. Seller agrees to deliver
marketable title at closing.
6. REPRESENTATION: Unless Seller and Listing Broker have entered into a separate representation
agreement, Listing Broker has a "No Brokerage Relationship" with the Seller. Listing Broker DOES
NOT represent the Seller as either a Single Agent nor as a Transactional Broker.

7. INDEMNIFICATION: Seller shall indemnify, defend and hold Listing Broker harmless from and
against any and all claims, demands, suits, damages, liability, losses or expenses (including reasonable
attorneys' fees) arising out of any misrepresentation, nondisclosure or concealment by Seller in
connection with the sale of the Property, including, without limitation, the inaccuracy or incompleteness
of any information provided by Seller for listing in the MLS.

8. UNDER LISTING AGREEMENT: (hereinafter known as the Agreement), the Seller grants Listing
Broker a Limited Service Listing on the property. Listing Broker will list the Seller's property on the
Multiple Listing Service (hereinafter known as the MLS) and will maintain a no-brokerage relationship
with the Seller. This means that while Listing Broker is paid to perform certain actions on behalf of the
Seller, Listing Broker does not represent the Seller. Should an unrepresented Buyer contract to
purchase said Property the Seller can sell their property themselves, in which case no commission is
due. Seller can cancel this Agreement at any time by notifying the Listing Broker in writing unless the
Seller has a contract in effect.

9. ACCESS TO DOCUMENTS: By signing this Agreement, Seller acknowledges that he/she has
access to Listing Broker for copies of all pertinent information such as change forms, Seller's
Disclosure Forms (these include the Lead-Based Paint Disclosure and EPA Lead-Based Paint
Pamphlet available on www.ajaflahome.com).

10. NO GUARANTEE: Seller understands that this Agreement does not guarantee the sale
of the Property.

11. SIGN: Must use our Real Estate sign (it says For Sale With Owner).

12. LOCK BOX:. Seller agrees that a lock box, if utilized, will be for the benefit of the Seller and releases
Listing Broker and those working by or through Listing Broker and the Association(s) of Realtors from all
liability and responsibility in connection with any loss or damage that may occur.


13. CHANGE FEE: Listing Broker will provide Seller with a copy of the active MLS listing once it is active
in the MLS database. At that time, corrections are allowed and will be made by Listing Broker's
administrative staff to reflect accurate information. After the final corrections have been made, all
changes to the MLS listing, other than status and price changes,i.e.,”Active to Pending or Pending to
Sold”, will incur a fee of $25.00 for each database access, at which time multiple changes may be made.
All corrections, updates and/or changes to the MLS listing must be made in writing to our administrative
department at: 819 Dunbar Ave, Oldsmar, FL 34677 or via fax to 813-854-2700. Verbal requests for
changes to the MLS listing will not be honored. Listing Broker reserves the right to suspend any and all
advertising, permanently withdraw the MLS listing, or withhold changes to the MLS listing at any time in the
event of default by Seller of any terms under this Agreement, including, but not limited to, nonpayment of
any monies due Listing Broker. Furthermore, if Seller fails to respond in a timely fashion to Listing Broker's
communication (including e-mail) regarding the status of their home sale and the status of their MLS listing,
Listing Broker reserves the right to remove the Seller's MLS listing from the MLS and all other forms of
advertising at any time during the MLS listing time period (per listing agreement).

14. THE MULTIPLE LISTING SERVICE (MLS), LISTING BROKER HELD HARMLESS: Seller
acknowledges that Listing Broker intends to rely solely upon the accuracy of all information furnished by
Seller and Seller agrees to indemnify and hold harmless Listing Broker and MLS, their agents and
employees, from any and all losses, costs, damages or liabilities of any nature whatsoever resulting from
Seller's withholding of any information from Listing Broker or as a result of Seller's providing any incorrect
information to the Listing Broker. Photography, internet advertising, i.e., www.Realtor.com and some
other forms of advertising are provided by the MLS (Multiple Listing Service), not the Listing Broker, and
Listing Broker is not responsible for any inaccuracies or clerical errors in MLS listings, any delays, errors
or interruption of services provided by MLS service, internet providers, internet advertising, signage or
other third party vendors, and Seller specifically indemnifies and holds Listing Broker harmless from any
and all claims, losses and/or liability arising from any matters involving MLS, photography, internet
advertising, delivery of materials or any other forms of advertising including errors or omissions in any
listing placed in the MLS, internet or other electronic or printed media.

       Listing Broker will:

       a. Place the Property in a Multiple Listing Service (MLS) from the listing date to the expiration
          date on this Agreement. For Plans II and III it will be one year (If not sold call to extend at no
          charge) before end of listing. Seller authorizes Listing Broker to report to the MLS this
          Property's information, price, terms and financing information on any resulting sale for use
          by authorized Board/Association members, MLS participants and subscribers.
       b. See addendum regarding lock boxes, their use and associated liability.
       c. Listing Broker will provide the MLS listing only. All inquiries, showings, contracts and
          negotiations shall be handled directly by the Seller without assistance from Listing
          Broker.


15. RIGHT TO INSPECT POLICY: All Flat Fee Listings and all property For Sale will be listed AS- IS with
Right to Inspect.

                        Limited Service Listing Agreement
This Limited Service Listing Agreement (“Agreement”) is between

You will need internet access (the faster the connection the better)

First have your images ready to go: saved on your computer
       Most digital cameras save pictures with a file extension of .jpg (you will see this on the end of the file name)
       We will notify you if the images are not ready for MLS use.
Open you email software and send to pictures@ajaflahomes.com -
    If your county is not Pasco, Pinellas or Hillsborough please revert back to Flat Rate
   Counties Available tab on website for email address to send your pictures.
Reference in the Subject line: Pictures for Your Address Here
In your email please reference which photo you want as your main picture
       (Usually the exterior picture of the property)
       Also include your property address and contact information in the body of the email
Attach the pictures as an attachment
       (Usually a paperclip is present and this is what you will click on to attach the pictures)
       Search your computer for the pictures you saved with the browse button and attach
Click Send and wait for it to be sent before closing


If you do not receive a notification email from ajaflahomes.com that your pictures were sent
successfully within 24 hours please contact our office.

Remember: If you do not include contact information we will not be able to contact you if there
are problems with your pictures. If you do not reference an address to the property we will have no

Use of a lock box

If you plan on using the lock box system, you must advise Listing Broker of this fact when completing your listing agreement
form.

Keep in mind that it is advantageous for Realtors to be able to show your home when their clients are available to see' it.
Since this is likely to occur at times when you are personally unavailable to unlock the doors, having a lock box accessible to
Realtors can be a valuable asset in the process of selling your home.

If you choose to purchase a combination style lock box, we will inform Realtors through the MLS that they need to call you
directly for the combination to the lock box to show the property.

Listing Broker is indemnified from any and all liability associated with having a lock box on your home (either electronic or
combination) and cannot be held liable for any losses and/or damages incurred from the Seller's decision to utilize a lock
box system.

Use a quality lock box. Lowe’s and other stores have quality lock boxes for sale.
