PE MAKE-UP RESEARCH PAPER OR PROJECT
IMPORTANT INSTRUCTIONS:
You have missed 5 or MORE physical education classes and to make up these class points (50 pts) you are now
required to complete EITHER a research paper or project on a health or sport-related topic. READ the following
options and requirements of each option below. READ the suggested topics for research (on the back). Choose which
option you would like to do and which topic and complete the form below. CUT THE FORM OFF AND ATTACH THE
FORM TO YOUR PAPER OR PROJECT SO IT IS VISIBLE TO THE TEACHER. All projects and papers are DUE NO LATER
THAN the official last day of the quarter. Projects or papers turned in after the quarter ends will not be accepted.
Please turn your project in to the PE teacher that is responsible for grading you.
Failure to complete your make-up work OR follow the instructions listed
WILL result in a non-basic PE grade for the quarter.
