									              The Chairman’s Sport Awards 2012
                         Guidelines
  About the Awards
  The impact of volunteers, coaches and teachers upon the sporting landscape cannot
  be overestimated. These awards will recognise the work of these individuals and
  organisations that are a key part of increasing participation, raising the level of
  people’s health and building stronger communities in Bath and North East Somerset.

  The Chairman’s theme for the year in office is: Recognising Achievement in the
  Community and nominations are particularly invited that link with this theme.

  All nominees must reside or be active in Bath and North East Somerset.

  Categories
  1. Young Volunteer in Sport (16 and under)
     This award will recognise overall commitment shown by a volunteer in community
     sport and should be an individual who has made a significant contribution to sport
     helping behind the scenes, as a coach, team manager or mentor.

  2. Volunteer in Sport
     This award will be presented to an individual (17 and over) in recognition of their
     commitment and achievements as a volunteer e.g. coach, team manager, chair
     person, secretary, fundraiser, team manager or mentor

  3. Young Achiever in Sport (16 and Under)
     This award will be presented to an individual who has demonstrated significant
     development and progress in their particular sport and has inspired others around
     them with their dedication, attitude and commitment.

  4. Achiever in Sport
     This award will be presented to an individual (17 and over) in recognition of their
     commitment and achievements within their sport. Nominations could mention levels
     of commitment, contributions made as a competitor/team member, personal
     development, mentoring and support of other participants.

  5. Achiever in Sport (with a disability)
     This award will be presented to an individual with a disability in recognition of their
     commitment and achievements within their chosen sport. Nominations should
     mention their levels of commitment, overcoming adversity, contributions made as a
     competitor/team member, personal development, mentoring and support of other
     participants.

  6. Coach
     This award will recognise overall commitment shown by a coach in community
     sport and physical activity and will consider commitment, contributions in their
     chosen sport, commitment to personal development, mentoring and support
     of others.

  7. Sports Club
     This award will be presented to the Amateur Community Sports Club that has
     shown the most commitment to developing community sport. Nominations will also
     be sought from NGB partners and will recognise level of activity, commitment,
     attitude and work towards accreditation.

  8. Teacher (PE and Sport)
     This award will be presented to a teacher in recognition of their involvement and
     dedication to providing sporting opportunities for young people. Nominations from
     young people themselves are welcome

  9. Young Sports Leader
     This award will be presented to a Young Sports Leader (18 and under) in
     recognition of their involvement and dedication to providing sporting opportunities
     for young people. Nomination should mention levels of involvement, commitment
     including lunchtime and after school, levels of experience and mentoring / support
     given to others

  10. Healthy Lifestyles Special Achievement Award
      This award will be presented to an individual, registered on any of the Bath and
      North East Somerset Council healthy lifestyles programmes including cycling,
      walking, running, Passport to Health and community activators, who has shown the
      most commitment and/or health improvement. Nominations could mention levels
      of activity, commitment, attitude, personal development and barriers overcome.

  11. Aquaterra Achievement Award
     This award will be presented to an Aquaterra member who has shown the most
     progress and commitment and/or health improvement. Nominations could mention
     levels of activity, commitment, attitude, personal development and barriers
     overcome.

  12. Chairman’s Special Award
     The Chairman will be presenting a ‘Special Award’ to an individual or group who
     reflects achievement in the Community. The Chairman will choose the winner of
     this category from the nominations received in the above categories.

  All nominees will be presented with a certificate in recognition of their contribution. The
  winners in each category will receive an award plus 12 months Platinum Membership
  to Aquaterra Leisure run facilities (worth £400) presented by the Chairman of the
  Council at an evening award ceremony on 13th March 2012.

  Deadline
  Nomination forms must be returned by no later than 12 noon on 3rd February 2012.

  Who can make nominations?
  Individuals, businesses, sports clubs, council staff, councillors, youth
  organisations, schools, FE/HE institutions, teachers, NGBs, CSPs
